Diz osteoartritli hastalarda quadriceps femoris kasına uygulanan iki farklı elektroterapi akımının etkilerinin karşılaştırılması

Abstract:

Amaç: Diz osteoartritli (OA) bireylerde quadriceps femoris kasına uygulanan Rus akımı ile Yüksek Voltaj Kesikli Galvanik stimülasyonun (YVKGS) kas kuvveti, fonksiyonel durum ve yaşam kalitesi üzerine etkilerini karşılaştırmaktır. Yöntem: Bilateral diz OA tanılı 26 hasta çalışmaya dahil edildi ve 2 gruba ayrıldı. Her iki gruba da 3 hafta boyunca haftada 5 gün 15 seans, hotpack, ultrason (US) ve egzersiz tedavisine ek olarak M. Quadriceps femoris kasına 1. grupta Rus akımı, 2. grupta ise YVKGS uygulandı. Tedavi öncesi ve sonrasında OA ile ilişkili özürlülük Western Ontario and McMaster Üniversitesi Osteoartrit İndeksi (WOMAC) ile, fonksiyonel mobilite Zamanlı Kalk-Yürü Testi (ZKYT) ile, yaşam kalitesi Kısa Form 36 (KF-36) anketi ile ve quadriceps femoris izokinetik kas kuvveti 90°/sn, 120°/sn, 180°/sn açısal hızlarda izokinetik sistem ile değerlendirildi. Bulgular: Her iki grupta WOMAC ağrı, fonksiyon, toplam ve ZKYT puanlarında, ayrıca 2. grupta WOMAC eklem tutukluluğunda anlamlı iyileşme saptandı (p<0,05). KF-36’da 1. grupta fiziksel rol, mental sağlık ve vücut ağrısında, 2. grupta ise emosyonel rol kısıtlılıkları ve vitalite bölümlerinde anlamlı iyileşme görüldü (p<0,05). İzokinetik değerlendirmede ise sadece 1. grupta sol dizde zirve tork değerindeki artış istatistiksel anlamlılık gösterdi (p<0,05). Sonuç: Diz OA tedavisinde uygulanan her iki elektrik stimülasyonu uygulamasının da bireylerde fonksiyon, mobilite ve yaşam kalitesini arttırmada etkili olduğu gösterilmiştir. Ancak Rus akımının kas kuvvetini arttırmada daha etkili olduğu görülmüştür. Bu konuda daha uzun süreli araştırmalara ihtiyaç vardır.

Purpose: The Russian flow applied to the quadriceps femoris muscle in individuals with knee osteoartritis (OA) is to compare the effects of High Voltage Cut Galvanic Stimulation (YVKGS) on muscle strength, functional condition and quality of life. Method: Bilateral knee OA diagnosed 26 patients were included in the study and divided into 2 groups. Both groups had 5 days a week of 15 sessions, hotpack, ultrasound (US) and exercise therapy in addition to M. Quadriceps femoris muscle, Russian flow in the 1st group and YVKGS in the 2nd group. Pre- and post-treatment disability related to OA with the Western Ontario and McMaster University Osteoarthritis Index (WOMAC), functional mobility with the Timely Rise and Walk Test (ZKYT), quality of life with the Short Form 36 (KF-36) survey and quadriceps femoris isocinetic muscle strength with the isocinetic system at 90°/s, 120°/s, 180°/s angle speeds. Results: WOMAC pain, function, total and ZKYT points were significantly improved in both groups, and WOMAC joint retention in group 2 was significantly improved (p<0,05). In KF-36, physical role in the 1st group, mental health and body pain, and in the 2nd group; significant improvement in emotional role limitations and vitality parts (p<0,05). The isocinetic assessment is only 1. The increase in the top turn value in the left knee in the group showed statistical significance (p<0,05). Result: Both electrical stimulation applications used in knee OA treatment have also been shown to be effective in improving function, mobility and quality of life in individuals. However, the Russian flow has been shown to be more effective in increasing muscle strength. This requires longer research.
