Amaç: Bu çalışma ile meme kanseri tanısı almış kadınlarda klinikopatolojik parametrelerin retrospektif olarak değerlendirilmesi ve moleküler alt tiplerin araştırılması amaçlandı. Materyal ve Metot: Çalışmamız kapsamında Kırıkkale Üniversitesi Tıp Fakültesi Tıbbi Patoloji Anabilim Dalı arşivi taranarak Ocak 2007-Haziran 2019 tarihleri arasında meme kanseri tanısı verilmiş 191 hasta incelendi. Bulgular: İncelenen histopatolojik tümör tipleri, meme tümörlerinin Dünya Sağlık Örgütü 2012 sınıflamasına göre, en yüksek oranda (%89) özel tip olmayan (NST, duktal) meme kanseri olgularına ait olduğu belirlendi. Tümör çapı bakımından en büyük grubu (%48,7) 2–5 cm arası olan olguların oluşturduğu anlaşıldı. Modifiye ScarffBloom-Richardson ile değerlendirilen meme kanseri derece 2 (%55,5) tümörler, alışmamızdaki en büyük grubu oluşturdu. Yaş grubu arttıkça tümör derecesinin arttığı istatistiksel olarak anlamlı düzeyde (p<0,001) belirlendi. Tümörlerde, hormon reseptörleri (ER ve PgR) ile yüksek oranda boyanma saptandı. Hastaların %61,2’sinde lenf nodu invazyonu belirlendi. Bölgesel lenf nodu evresi (pN) 0 olan hastalar %38,8 ile en büyük grubu oluşturdu. Moleküler alt tip bakımından ise en çok (%48,2) luminal A (LA) tipi izlendi. Bunu luminal B (LB) (%37,4), HER2 overeksprese (HO) (%10,8) ve triple negatif (TN) (%4,6) oranları takip etti. LB’nin, LA’ya göre histolojik tümör derecesi bakımından daha ileri olduğu (p<0,001) anlaşıldı. Tümör derecesi HO ve TN’de LA’ya göre daha ileri (p<0,005) düzeyde saptandı. Tümör çapı bakımından ise HO, LA’ya göre yüksek düzeyde tespit (p<0,05) edildi. TN’de HO’ya göre daha büyük bir yaş düzeyi (p<0,05) saptandı. Sonuç: Heterojen bir tümör olan meme kanserinde, tümör derecesi en önemli prognostik faktörlerden biridir. Çalışma verilerimiz bir arada değerlendirildiğinde, meme kanseri moleküler alt tiplerinin belirlenmesinin rutin histopatolojik analiz için önemli yararlar sağlayacağı kanısına varılmıştır
Purpose: This study aimed at the retrospective assessment of clinic pathological parameters in women diagnosed with breast cancer and the study of molecular subtypes. Materials and Methods: In the framework of our study, 191 patients diagnosed with breast cancer were examined by scanning the archive of the Medical Pathology Department of the University of Kırıkkale Medical Faculty in January 2007–June 2019. Results: The examined histopathological tumor types found that breast tumors according to the World Health Organization’s 2012 classification, the highest rate (89%) belonged to non-specific type (NST, duktal) breast cancer occurrences. It was found that the largest group of tumor diameter (48.7%) consisted of events between 2-5 cm. Modified with ScarffBloom-Richardson, breast cancer grade 2 (55,5%) tumors formed the largest group we used to. The age group increased and the tumor rate increased at a statistically significant level (p<0,001). In tumors, high levels of coloring were detected with hormone receptors (ER and PgR). In 61.2% of patients, lymphatic nod invasion was identified. Patients with a regional lymph nod stage (pN) 0 formed the largest group with 38.8%. The molecular subtype was the most observed (48.2% of the luminal A (LA) type. This was followed by luminal B (LB) (37.4%, HER2 overexpress (HO) (10.8) and triple negative (TN) (4.6). It was found that LB was higher in terms of the histological tumor degree (p<0,001) compared to LA. The tumor rate was determined at a higher (p<0,005) level in HO and TN than in LA. In terms of tumor diameter, HO was detected at a high level (p<0,05) compared to LA. In TN, a greater age level (p<0,05) was identified compared to HO. In breast cancer, which is a heterogeneous tumor, the tumor degree is one of the most important prognostic factors. When our study data is evaluated together, the determination of molecular subtypes of breast cancer has been found to provide significant benefits for routine histopathological analysis.
