Abstract:

Kedi tırmığı hastalığı (KTH), Bartonella henselae tarafından oluşturulan, kendini sınırlayan ve benign seyirli bir hastalıktır. KTH, enfeksiyöz lenfadenopatilerin önemli bir nedenidir ve en sık olarak aksiller, servikal, preaurikuler ve submandibuler lenf nodu gruplarını tutar. Enfeksiyon genellikle bir kedi ile temas öyküsü bulunan çocuklarda ve genç erişkinlerde görülür. Burada daha önce sağlık sorunu bulunmayan, antibiyotik tedavisine yanıt vermeyen submandibuler bez enfeksiyonu ve pürülan akıntısı olan 42 yaşındaki bir kadın hasta sunulmaktadır. Submandibuler bezin cerrahi eksizyonu ile enfeksiyon iyileştirilmiş ve histopatolojik inceleme bulgularına göre KTH tanısı konmuştur. Anahtar Kelimeler: Kedi tırmığı hastalığı, submandibuler bez, tükrük bezi hastalıkları, lenfadenit Sialadenitis and Salivary Gland Abscess Refractory to Medical Treatment: An Atypical Presentation of Cat Scratch Disease Cat scratch disease (CSD) is a benign and self-limiting infectious disease caused by Bartonella henselae. CSD is an important cause of infectious lymphadenopathies and the most common involvement sites are axillary, cervical, preauricular and submandibular lymph node groups. The infection usually occurs in children and young adults who have a history of contact with cats. In this report, we present a previously healthy 42-year-old female presenting with submandibular gland infection and purulent drainage resistant to antibiotics. Infection was succesfully cured after surgical excision of the submandibular gland and the diagnosis of CSD was made on the basis of histolopathological findings of the surgical specimen.Key Words: Cat-scratch disease, submandibular gland, salivary gland diseases, lymphadenitis
