Abstract:

Dieulafoy’s lesion is a rare cause of severe lower gastrointestinal hemorrhage. Although usually found in the stomach, the lesion may occur anywhere within the gastrointestinal tract. Prompt endoscopic examination and possibly multiple repeated endoscopies may be needed to make the diagnosis. The current therapy of choice is endoscopic combined therapy. We describe a female patient who presented with massive lower gastrointestinal bleeding caused by a colonic Dieulafoy’s lesion. The source of the bleeding was suspected to be in the right colon by colonoscopy. Our case also experienced recurrent bleeding; endotherapy failed to achieve hemostasis and lapsed into hypovolemic shock. She was taken up for emergency surgery with right hemicolectomy. The histological sections of this lesion revealed an ulceration in the colonic mucosa which covered a number of irregular and dilated vascular structures with varying wall thicknesses
