Akut Kolesistit ile Başvuran Atipik Bruselloz Olgusu

Ülkemizde endemik zoonotik hastalık olan bruselloz çeşitli klinik tablolar ve komplikasyonlara karşımıza çıkabildiğinden tanı ve tedavisinde zorluk yaşanabilen bir hastalıktır. Bakteriyemi yapabilme özelliği nedeniyle Brucella cinsi bakteriler  çeşitli organlara yerleşip  atipik formlarda karşımıza çıkabilir. Brucella türlerinin neden olduğu kolesistit  nadir görülen bir tablodur. Bu makalede, tanısında oldukça zorlanılan, tanısı dren kültüründe Brusella melitensis üremesi ile konulan seronegatif bruselloza bağlı akut kolesistit olgusu sunuldu. Akut kolesistit ön tanısı ile opere edilen postoperatif dönemde ateş yüksekliği ve lökopeni gelişen hastada, negatif Brusella serolojisine rağmen kan kültüründe ve dren kültüründe Brucella melitensis tespit edilmiştir. Hasta 6 haftalık doksisiklin 2x100 mg/gün ve rifampisin 1x600 mg/gün tedavisi sonrası sekelsiz iyileşmiştir.

Atypic Brussels disease with acute cholecystitis

Brusellosis, an endemic zoonotic disease in our country, is a disease that can be difficult in diagnosing and treating because it can face various clinical tables and complications. Because of its ability to make bacteria, Brucella-like bacteria can settle in various organs and appear in atypical forms. The cholecystitis caused by the Brucella species is a rare picture. In this article, the symptoms of acute cholecystitis associated with seronegative bruselloza, diagnosed with the reproduction of Brusella melitensis in the dren culture, which was quite difficult to diagnose, were presented. In the postoperative period with the pre-diagnosis of acute cholecystitis, in patients developing high fever and locopenia, Brucella melitensis was detected in the blood culture and dren culture despite the negative Brussels serology. The patient was healed after 6 weeks of doxycycline 2x100 mg/day and rifampinine 1x600 mg/day treatment.
