Amaç: Bası yarası, dokuların uzun süreli basınç altında kalmasına bağlı olarak gelişen ve daha çok vücudun kemik çıkıntılarının üzerinde gözlenen iskemik zeminde ortaya çıkan yaralardır. Evre 4 bası yarası gelişmiş olgularda çoğu zaman cerrahi tedavi en uygun seçenek olarak karşımıza çıkar. Bu çalışmada pelvik bölgede evre 4 bası yarası nedeni ile kliniğimizde tedavi edilen hastaların değerlendirmesini ve bası yaralarına cerrahi yaklaşımımızı sunmayı amaçladık. Yöntem: Ocak 2008-Ocak 2012 tarihleri arasında kliniğimizde pelvik bölgesinde bası yarası tanısı ile yatarak tedavi görmüş 23 olgu retrospektif olarak değerlendirildi. Olguların etiyolojik ve epidemiyolojik bilgileri incelenerek uyguladığımız tedavi yöntemlerinin etkinliği irdelendi. Bulgular: Olguların 16’sı erkek, 7’si kadındı ve yaşları 17 ile 80(ortalama: 41) arasında değişmekteydi. Tüm olgularda Evre 4 bası yarası olmakla birlikte mevcut yaranın ilk meydana geliş zamanından itibaren geçen süresi 15 gün ile 12 ay (ortalama: 3,4 ay) arasında değişmekteydi. Opere edilen 18 adet olguda toplam 21 adet defekt onarıldı. Defektin lokalizasyonu ve boyutuna göre çeşitli flepler planlanarak onarımı sağlandı. Komplikasyon olarak üç olguda sütür hatlarında ayrışma, bir hastada yara yeri enfeksiyonu ve bir hastada parsiyel flep nekrozu görüldü. Sonuç: Bası yaraları, immobil hastalarda yaşamlarının en azından bir döneminde görülmekle birlikte ilerlemiş bası yaralarında cerrahi tedavi en etkin tedavi olarak öne çıkmaktadır. Cerrahi tedavi uygulanacak olgularda hastanın durumu ve iyileşme beklentisi, gelişmesi muhtemel komplikasyonlar göz önüne alınarak olgu için en uygun seçenek uygulanmalıdır.
Objective: The bacterial wound is the wounds that develop due to the long-term presence of the tissues and occur more on the ischemic ground observed on the bone marrow of the body. In advanced phases, phase 4 press wounds often come to us as the most suitable option for surgery. In this study, we aimed to evaluate patients treated in our clinic with the cause of stage 4 press wounds in the pelvic area and provide our surgical approach to press wounds. Method: between January 2008 and January 2012 23 facts in our clinic were considered retrospective with a press wound diagnosis in the pelvic area. The effectiveness of the treatment methods we use by studying the etiological and epidemiological information of the incidents. Results: 16 of the cases were male, 7 female, and their ages varied between 17 and 80 (medium: 41). In all cases, the phase 4 was a press wound, but the period from the time of the first occurrence of the current wound varied from 15 days to 12 months (medium: 3.4 months). In the 18 cases, a total of 21 defects were corrected. Depending on the localization and size of the defect, various fleets were planned to be repaid. As a complication, there were three cases of separation in the slurry lines, infection in the wound place in a patient and partial flep necrosis in a patient. The result is that the surgical wounds are seen in immobile patients at least in one period of their lives, while the surgical treatment of advanced press wounds is the most effective treatment. In the circumstances in which surgery is to be applied, the patient’s condition and the expectation of recovery, taking into account the possible complications of his development, should be applied the most suitable option for the occurrence.
