Abstract:

Amaç: Günümüzde operasyonlarda kullanılan sütür materyalleri çok ge- lişmiş olmalarına rağmen bazen kullanım tekniğine bağlı olarak, bazen de hastadan kaynaklanan nedenlerle birtakım komplikasyonları beraberinde getirebilmektedir. Cerrahide kullanılan absorbable sütürlerin nonabsor- bable olan sütürlere göre yara yerinde daha az reaksiyona neden olduğu gösterilmiştir. Bu çalışmada sternotomi sonrası cilt ve cilt altı polypropy- lene sütür ile kapatılan ve dikiş reaksiyonu gelişerek sonradan yarası açı- lan 8 hasta retrospektif olarak incelendi. Materyal ve Metod: Hastalar Mart 2002 ile Şubat 2004 tarihleri arasında opere edildiler. Yaş ortalamaları 60,7±22,4 idi. Bu hastalar postoperatif en erken 30. günden itibaren değişik sürelerde yaralarının kapanmaması ve ağrı yakınmalarıyla polikliniğimize başvurdular. Yapılan ilk muayene- de sternotomi cilt insizyon yerlerinde yara iyileşmesinin tam olmadığı ve yara yerlerinde kızarıklık, açılma, seröz bir akıntı olduğu görüldü. Has- taların tamamında polypropylene dikiş kullanıldığı ve bu dikişlerin yer yer açıkta olduğu görüldü. Yara yeri kültürleri alındı ve cerrahi girişimle nonabsorbable polypropylene sütürler absorbable sütür materyali ile de- ğiştirildi. Bulgular: Tüm kültürlerin sonuçlari negatifti ve absorbable sütür mater- yali kısa sürede reaksiyonsuz yara iyileşmesi sağladı. Sonuç: Polypropylene sütürler yara iyileşmesi süresinde cilt reaksiyon- larına neden olabilir. Absorbable sütür materyalleri polypropylene sütür materyali kullanımına bağlı bu tür cilt reaksiyonlarında uygun alterna- tiflerdir. Anahtar Kelimeler: Polypropylene Sütür, Sütür Reaksiyonları, Sternal Cilt İnsizyonu,Yara iyileşmesi   SKIN REACTIONS AGAINST POLPROPYLENE SUTURE MATERIALS USED IN STERNAL INCISION Aim: Suture materials used today are highly developed. However, they may still lead to complications associated with the technique used and patient-related factors. Absorbable sutures used in surgical interventions have been shown to lead to fewer reactions than nonabsorbable sutures. This retrospective study evaluated 8 patients who developed reactions to polypropylene sutures used for subdermal tissue and skin closure. Materials and Methods: Eight patients (mean age: 60.7±22.4 years) with inappropriate wound healing and pain at the wound site were admitted to our hospital between March 2002 and February 2004. The patients had sought treatment at the outpatient clinic with the complaints of insuffici- ent wound healing and pain as early as the postoperative 30th day. The ini- tial examination revealed that wound healing in sternal incision areas was not complete, and redness, opening and serous leakage were observed. All the patients had received polypropylene sutures, which were visible in certain areas. Wound cultures were taken and polypropylene sutures were substituted with absorbable suture materials by surgical intervention. Results: All the cultures were negative, and absorbable suture materials conveyed non-reactional wound healing in a relatively shorter time. Conclusion: Polypropylene sutures lead to skin reactions during wound healing. Absorbable suture materials are appropriate alternatives for those who suffer skin reactions due to polypropylene suture material use.
