Azigos ile Devam Eden İnferior Vena Kava’ya Eşlik Eden Polispleni ve Retroaortik Sol Renal Ven

Abstract:

İnferior vena kava (İVK) anomalileri genel populasyonda nadirdir sol renal ven anomalileri de düşünüldüğünde, %8,7’ye kadardır (Yang, Trad, Mendonça, Trad, 2013). Kesitsel görüntülemenin gelişimi ve ulaşılabilirliği ile asemptomatik populasyonda İVK ve dallarının anomalilerinin gösterilebilmesi kolaylaşmış ve saptanma sıklığı artmıştır. Olgumuzda 67 yaşında bir kadın hastada, insidental olarak İVK’nın intrahepatik kesiminin bulunmadığı ve İVK’nın retrokrural bölgede genişlemiş azigos veni şeklinde devam ettiği bunun yanında, polispleni, çift sağ renal ven ve retroaortik sol renal venin de tabloya eşlik ettiği gözlendi. İVK anomalilerinin tanınmasında, İVK’nın majör anomalilerinin basit embriyonik temellerinin bilinmesi doğru yorumlama ve raporlama için gereklidir. İnvaziv prosedürler öncesi anomalilerin tanınması olası komplikasyonların engellenmesinde yardımcı olabilir. Görüntüleme yöntemlerinin artan kullanımı ile sıklıkla asemptomatik hastalarda rastlansa da açıklanamayan erken erişkinlik çağı trombozlarında, kronik venöz yetmezliklerde İVK anomalileri akılda bulundurulmalıdır.

Azygos Continuation Of Inferior Vena Cava Associated With Polysplenia and Retroaortic Left Renal Vein

Inferior vena cava abnormalities(IVC) are very rare in general population and up to 8.7% including left renal vein abnormalities (Yang, Trad, Mendonça, Trad, 2013). With the accessibility and the development of cross-sectional imaging, abnormalities of IVC and its branches can be easily depicted and the frequency rate has been increased. In our case, a 67-year-old female patient had imaging findings of absence of intrahepatic segment of IVC and its continuity with dilated azygos vein, which were seen incidentally, in association with polysplenia, double right renal vein and retroaortic left renal vein. Knowing the embryonic basics of the major abnormalities of IVC is necessary for being aware of the IVC abnormalities and reporting and interpreting them accurately. To be aware of the abnormalities before invasive procedures can help preventing the possible complications. Although due to increased use of the imaging modalities, the IVC abnormalities are frequently seen in asymptomatic patients, one should also be kept in mind the likelihood of their presence in the patients with early adulthood onset thrombosis and chronic venous insufficiency.
