Amaç: Normal over yanıtlı hastalarda in vitro fertilizasyon (IVF) sırasında GnRH-agonist ve GnRH-antagonist tedavinin embriyo kalitesine ve insan koryonik gonadotropin (hCG) günü bakılan E2 ve PRL düzeylerine olan etkisine bağlı olarak gebelik sonuçlarına etkisi değerlendirilmiştir. Gereç ve Yöntemler: Kontrollü ovaryan hiperstimülasyonda GnRH agonist (n=44) veya GnRH (n=71) antagonist ile down regülasyonu yapılarak oosit toplanması, intrastoplazmik sperm injeksiyonu (ICSI) ve embriyo transferi yapılan 18-40 yaş arası 115 hastada prospektif olarak çalışılmıştır. hCG yapılma günü ve bazal serum E2 ve PRL düzeyleri ölçülmüştür. Demografik bilgiler, over cevabı, endometriyal kalınlık, embriyo gelişimi, ve IVF sonuçları her iki grup arasında karşılaştırılmıştır. Bulgular: Tedavi protokolleri sonucunda elde edilen oosit sayısı, matür olan oosit sayısı ve fertilize olan oosit sayısı istatiksel olarak farklı değildir. hCG günü ultrasonografik olarak bakılan endometriyal kalınlık ve endometriyal görünüm, serum E2 (1818±1064 pg/ml vs 1592±860 pg/ml, sırasıyla, p>0.05) ve PRL (34.2±20.1 ng/ml vs, 30.3±15.4 ng/ml, sırasıyla, p>0.05) düzeyleri iki grup arasında istatiksel olarak farklı değildir. GnRH-agonist ve GnRH-antagonist tedavi siklusları sonucunda fertilizasyon oranı (60.3% ve 60.9%, sırasıyla, p>0.05), implantasyon oranı (20.5% ve 23.5%, sırasıyla, p>0.05), gebelik oranı (27.3% ve 21.1%, sırasıyla, p>0.05) istatiksel olarak farklı değildir. Gebelik oranları için hCG günü bakılan E2 ve PRL AUC düzeyleri için yapılan ROC eğim analizi sonuçları IVF/ICSI sikluslarında gebelik oluşumunu tahmin etmede kullanılamayacağı görülmüştür (AUC 0.505, 95% CI: 0.372–0.638; AUC 0.412, 95% CI: 0.286-0.537, sırasıyla ). Sonuçlar: GnRH-agonist veya GnRH-antagonist tedavi protokolü uygulanan normal over yanıtlı hastalarda bu tedavi farklılığının embriyo kalitesi üzerine etkisi olmadığı ve gebelik sonuçlarını etkilemediği görülmüştür. GnRH-agonist ve antagonist sikluslarda hCG günü bakılan E2 ve PRL ortalama kan düzeyleri farklı bulunmamıştır. Ayrıca hCG günü bakılan E2 ve PRL düzeylerinin gebelik oluşumunu tahmin etmede kullanılmasının anlamlı olmadığı görülmüştür.
The effect of GnRH-agonist and GnRH-antagonist therapy during in vitro fertilization (IVF) in normal over-responsive patients was assessed depending on the embryonic quality and the effect of human coronary gonadotropin (hCG) on the E2 and PRL levels considered on the day. In controlled ovarian hyperstimulation GnRH agonist (n=44) or GnRH (n=71) antagonist with down regulation of oocyte collection, intrastoplasmic sperm injection (ICSI) and embryo transfer were prospectively studied in 115 patients aged 18-40 years. The day of making of hCG and the basal serum E2 and PRL levels are measured. Demographic information, over response, endometrial thickness, embryonic development, and IVF results are compared between both groups. Results: The number of oocytes obtained as a result of the treatment protocols, the number of oocytes that are matures and the number of oocytes that are fertilized are not statistically different. The endometrial thickness and endometrial appearance considered ultrasonically on hCG day, the serum E2 (1818±1064 pg/ml vs 1592±860 pg/ml, respectively, p>0.05) and PRL (34.2±20.1 ng/ml vs, 30.3±15.4 ng/ml, respectively, p>0.05) levels are not statistically different between the two groups. The fertilization rate as a result of GnRH-agonist and GnRH-antagonist treatment cycles (60.3% and 60.9%, respectively, p>0.05), the implantation rate (20.5% and 23.5%, respectively, p>0.05), the pregnancy rate (27.3% and 21.1%, respectively, p>0.05) are not statistically different. The results of the ROC curve analysis for the E2 and PRL AUC levels considered for pregnancy rates on hCG day have not been found to be used to predict pregnancy in IVF/ICSI cycles (AUC 0.505, 95% CI: 0.372-0.638; AUC 0.412, 95% CI: 0.286-0.537, respectively ). Results: In normal over-response patients with GnRH-agonist or GnRH-antagonist treatment protocols, this difference in treatment has not been shown to affect the quality of the embryos and does not affect the result of pregnancy. In GnRH-agonist and antagonist cycles, the average blood levels of E2 and PRL treated on hCG day did not differ. It has also found that it is not meaningful to use the E2 and PRL levels considered on hCG day to predict pregnancy formation.
