Metilprednizolon ve Meloksikamın İntraabdominal Adezyonların Önlenmesi Üzerine Etkileri

Bu çalışmanın amacı, non steroid anti inflamatuar (NSAİİ) bir ilaç olan meloksikam ile steroid bir ajan olan metilprednizolonun ratlarda intraabdominal adezyonların önlenmesi üzerine olan etkilerini karşılaştırmaktı. Çalışmada 21 adet 2-3 aylık dişi Wistar albino rat kullanıldı. Ratlar her grupta 7 adet olacak şekilde 3 gruba ayrıldı. İki cm'lik laparatomi sonrasında karın duvarı ve sekumun visseral yüzeyinde serosal kanama oluşana kadar kazıma işlemi yapılarak bir yapışma modeli oluşturuldu. Operasyon yaraları rutin cerrahi kurallara göre kapatıldı. Birinci, ikinci ve üçüncü gruptaki ratlara 5 gün süreyle intraperitoneal olarak sırasıyla 7 mL/kg serum fizyolojik (kontrol), 5 mg/kg meloksikam ve 10 mg/kg metilprednizolon verildi. Ratlar ondört gün sonra ötenazi edildikten sonra oluşan adezyonlar değerlendirildi. Makroskopik değerlendirmede tedavi gruplarında adezyon oluşumu kontrol grubuna göre daha az olarak gözlendi (P=0.016). Yapılan histopatolojik kontrollerde, yangı şiddeti kontrol grubunda daha belirgindi. Fibrozis en belirgin olarak kontrol grubunda gözlendi (P=0.016). İmmunohistokimyasal değerlendirmelerde IL-1 pozitif hücre sayısının kontrol grubunda tedavi gruplarına göre daha fazla olduğu belirlendi (P=0.016). Sonuç olarak intraabdominal adezyonların önlenmesinde meloksikam ve metilprednizolonun kullanılabileceği kanısına varılmıştır.

Abstract:

The aim of this study was to compare the effects of methylprednisolone, a non-steroidal anti-inflammatory drug (NSAID) and a steroid agent, on the prevention of intraabdominal adhesions in rats. In the study, 21 of the 2-3-month female Wistar albino rat were used. The rats were divided into three groups in each group to be seven. After a two-cm laparatomy, a adhesive model was created by making a cutting process until serous bleeding occurs on the abdominal wall and secular surface. The wounds were closed according to the routine surgical rules. Rats in the first, second and third groups were given intraperitoneally for 5 days, respectively, 7 mL/kg of serum physiological (control), 5 mg/kg of meloxicam and 10 mg/kg of methylprednisolone. The rocks were evaluated after fourteen days after being overwhelmed. In the macroscopic assessment, the formation of adezyon in the treatment groups was less observed than in the control group (P=0.016). In the histopathological controls, the severity of the fire was more evident in the control group. Fibrosis was most apparently observed in the control group (P=0.016). Immunohistochemical assessments found that the number of IL-1 positive cells in the control group was greater than in the treatment groups (P=0.016). As a result, it has been found that meloxicam and methylprednisolone can be used in the prevention of intraabdominal adhesions.
