Herediter multipl ekzostoz (HME), uzun kemiklerin jukstaepifizer bölgesinden köken alan ekzostozlarla karakterize, otozomal dominant geçişli bir sendromdur. Temel defekt bozulmuş enkondral kemik oluşumu ve enkondral kemikleşmenin olduğu jukstaepifizer bölgelerde çok sayıda benign kemik aşırı büyümesidir (ekzostoz veya osteokondrom). İnsidansı 1/50.000’dir. Klinik olarak asıl yakınma eklem yakınında tek veya çok sayıda sert, ağrısız kitlelerin varlığıdır. Bu osteokondromlar büyümede geriliğe, ağrılı lokal semptomlara, damar, tendon, sinir basılarına, eklem hareketlerinde kısıtlılığa ve nörolojik bulgulara da neden olabilir. Burada göğüs ağrısı şikayeti ile göğüs hastalıkları polikliniğine başvuran ve akciğer grafisinde soliter pulmoner nodül tespit edilen HME tanısı olan hasta, soliter pulmoner nodülün ayırıcı tanısında nadir görülen bir neden olarak sunuldu.

Abstract:

Herediter multiple exostosis (HME) is an autosomal dominant transitional syndrome, characterized by exostosis that originate from the jukstaepifizer area of long bones. The main defect is the excess growth of a large number of benign bones (exostosis or osteochondrom) in the jukstaepifizer areas where the endocondral bone formation is deteriorated and the endocondral bone formation is deteriorated. The population is 1/50,000. Clinically, the main approach is the presence of one or more severe, painless masses near the joint. These osteochondroms can also cause growth tension, painful local symptoms, veins, tendons, nerve pressures, limitations in joint movements and neurological findings. Here, the patient, who was diagnosed with HME with a breast pain complaint with the clinic of breast diseases and identified solitary pulmonary nodes in the lung graph, was presented as a rare cause in the distinctive diagnosis of solitary pulmonary nodes.
