Amaç: Overin germ hücreli tümörleri arasında en sık görülen tip teratomlardır. Dermoid kist (matür kistik teratom) ise teratomların %95’ini oluşturur ve çoğunlukla benigndir. Fakat bu tümörlerde de %0,2-2’si malign transformasyon izlenebildiği için ayırıcı tanıda sıklıkla tümör belirteçleri kullanılır. Nötrofil lenfosit oranı (NLR) kişinin enflamatuvar durumunu belirlemek için kullanılan bir parametredir. Biz çalışmamızda tümör çapı, tümör belirteçleri, kist boyutu ve NLR parametrelerini kullanarak dermoid kistlerin ayırıcı tanısında enflamatuvar sürecinde değerlendirip değerlendirilmeyeceğini ve tümör belirteçlerinin dermoid kistlerdeki değişimlerini inceledik. Yöntem: Çalışmamıza retrospektif olarak 2017-2019 yılları arasında opere edilip, patoloji sonucu ile teyit edilen 14-45 yaş aralığınd 51 hastayı dahil ettik. Hastaların yaş, preoperatif hemogram, alfafetoprotein (AFP) (ng/mL), Ca (kanser antijeni) 125 (U/mL), Ca15-3 (U/mL), Ca19-9 (U/mL), karsinoembiryonik antijen (CEA) (ng/mL), kist boyutu (cm), NLR ve uni/bilateral oluşu kaydedildi. Bulgular: Olguların Ca 19-9 ortalama değerleri referans değerin üstünde bulunmuştur. Ca 19-9 değişkeni normal ve referans aralığından yüksek olarak iki gruba ayrıldı. AFP, Ca125 ve Ca15-3 kimyasal sonuçlarının gruplara göre anlamlı düzeyde bir farklılık gösterdiği bulundu. (Sırasıyla p=0.005, 0.029 ve 0.030). NLR, CEA ve kist boyutunun ise gruplarda benzer olduğu bulundu. Kist boyutuna göre değişkenlerin kıyaslanması sonuçlarına göre kist boyutu arttıkça hastaların yaş ortalamalarının anlamlı düzeyde azaldığı gözlendi (p=0.005). Ayrıca kist boyutu arttıkça Ca125 değerleri anlamlı olarak artış gösterdiği izlendi(p=0.001). Sonuç: Vakaları Ca 19-9 yüksekliğine göre ayırdığımızda Ca 19-9 yüksekliği ile korole olarak AFP, Ca125 ve Ca 15-3 değerlerinin anlamlı derecede yüksek bulunduğunu ortaya koyan literatürdeki ilk çalışma bizim çalışmamızdır.
Purpose: Overin germ cell tumors are the most common type of teratoma. Dermoid cysts (maturic cystic teratoms) make up 95% of the teratoms and are mostly self-induced. However, in these tumors, 0.2-2% of malignant transformations can be observed, so distinctive diagnosis is often used with tumor indicators. The neutrophil lymphocytes rate (NLR) is a parameter used to determine the inflammatory state of a person. In our study we studied whether the distinctive diagnosis of dermoid cysts by using tumor diameter, tumor indicators, cyst size and NLR parameters will be assessed in the inflammatory process and the changes in tumor indicators in dermoid cysts. Method: We included 51 patients from the age range of 14-45 who were operated retrospectively between 2017-2019 and confirmed by the pathology result. The patient’s age, preoperative hemogram, alphafetoprotein (AFP) (ng/mL), Ca (cancer antigen) 125 (U/mL), Ca15-3 (U/mL), Ca19-9 (U/mL), carcinomembrionic antigen (CEA) (ng/mL), cyst size (cm), NLR and uni/bilateral formations were recorded. Results: The average values of Ca 19-9 were found above the reference value. The Ca 19-9 variable was divided into two groups higher than the normal and reference range. The chemical results of AFP, Ca125 and Ca15-3 were found to be significantly different from groups. p = 0.005, 0.029 and 0.030 respectively. NLR, CEA and cyst size were found similar in groups. According to the comparison of the variables according to the cyst size, the patient’s average age has been significantly decreased (p=0.005). It was also observed that as the cyst size increased, the Ca125 values significantly increased (p=0.001). The result: When we divide cases according to the height of Ca 19-9 is our first study in literature that shows that the values of AFP, Ca125 and Ca 15-3 are significantly high as corolated with the height of Ca 19-9 is our work.
