Abstract:

Purpura Fulminans is a disease with high mortality and morbidity, seen with hemorrhagic necrosis of the skin and soft tissue in the limbs. The development of contracture in the joint areas results in loss of function in the limbs. The use of wound care products containing silver in the repair of skin defects, the primary dermal matrix and the partial thickness of the skin injection applications are among the current plastic surgical approaches. In our pediatric phenomenon with a 12-year-old diagnosed purpura fulminans with 55% body surface area necrosis, the skin equivalent MatriDerm was first applied in the repair to maintain the joint function with serial debridments and silver-containing wound coverings. The patient's lower end joint movements in extension and flektion in the ROM range of motion have not been detected.

Using silver-coated wound care dressings and acellular dermal matrix with split-thickness skin grafts to repair skin defects are the current approaches in plastic surgery. We used the acellular dermal substitute MatriDerm for the first time to protect joint movements in a 12-year-old pediatric patient diagnosed with Purpura Fulminans with necrosis of 55% of the total body surface area, after following the patient with extremity-protecting serial debridements and applying silver-containing wound dressings.
