Hemşirelik Birinci Sınıf Öğrencilerinin Palyatif Bakımda İlk Klinik Uygulama Deneyimleri

Abstract:

Amaç: Bu araştırmanın amacı, hemşirelik bölümü birinci sınıf öğrencilerinin palyatif bakım ünitesindeki deneyimlerini incelemektir. Yöntem: Çalışma, nitel araştırma yöntemleri kapsamında yer alan fenomenoloji deseni kullanılarak gerçekleştirilmiştir. Araştırmanın örneklemini, araştırmaya katılmayı kabul eden öğrencilerden amaçlı örnekleme yöntemlerinden olan benzeşik (homojen) örnekleme yöntemi kullanılarak belirlenmiş yedi öğrenci oluşturmuştur. Çalışmanın gerçekleştirilmesi için gerekli izinler alınmıştır. Araştırma verilerinin toplanmasında “Öğrenci Tanıtım Formu” ve “Yarı Yapılandırılmış Görüşme Formu” kullanılmıştır. Görüşmeler ortalama 45 dakika sürmüştür. Görüşmelerin ses kayıtları araştırmacılar tarafından çözümlenmek üzere belgelenmiş, elde edilen verilere onları açıklayabilecek kavramlara ve ilişkilere ulaşmak amacıyla araştırmacı tarafından içerik analizi  uygulanmıştır. Bulgular: Araştırma kapsamına alınan yedi hemşirelik birinci sınıf öğrencisinden dördü kız, üçü erkektir ve yaş ortalaması 20,57’dir. Verilerin içerik analizi doğrultusunda “hastaya ilişkin deneyimler”, “hemşirelik girişimlerine ilişkin deneyimler”, “yaşama ilişkin deneyimler” ve “hemşirelik mesleği algısına ilişkin deneyimler” olmak üzere dört tema belirlenmiştir. Temalar doğrultusunda ise “korku, umutsuzluk, ümitsizlik, öfke, zorluk, acı çekme, zamanın değerliliği, umut aşılama, mesleki özerkliğin anlamı, hemşire anahtar rol, insan onuru koruyucusu” olmak üzere 11 alt tema saptanmıştır. Sonuç: Hemşirelik öğrencilerinin ilk klinik deneyimlerini yaşadıkları palyatif bakımda hasta kavramına ve mesleklerine ilişkin yaşadıkları deneyimlerden, hastaların bağımlılık düzeylerinin yüksek oluşu ve hastaların içinde bulundukları tedavi ve bakım sürecinin hassasiyetine ilişkin düşünsel derinlik yaşadıkları ortaya konmuştur.

Purpose: The purpose of this research is to study the experience of first-class students in the palliative care unit of the nursing department. Method: The study was carried out using the phenomenological design within the framework of the quality research methods. The sampling of the study was formed by seven students identified using the similar (homogeneous) sampling method, which is a method of sampling intended from students who accepted to participate in the study. The necessary permits for the work were granted. The research data was collected using the "Student Promotion Form" and the "Semi-Structured Interview Form". The interviews lasted an average of 45 minutes. The audio recordings of conversations are documented to be analyzed by the researchers, the obtained data is implemented by the researchers with the aim of achieving concepts and relationships that can explain them. Results: Four of the seven nursing first-class students included in the study are girls, three men, and the average age is 20.57 years. According to the content analysis of the data, four topics have been identified: "patient-related experiences", "inheritance-related experiences", "life-related experiences" and "inheritance-related experiences". The theme is "Fear, desperation, desperation, anger, difficulty, suffering, the value of time, the vaccine of hope, the meaning of professional autonomy, the key role of nurses, the protector of human dignity." The first clinical experience of nursing has shown that patients experience the concept of palliative care and their professions, the high level of addiction of patients and the sensitivity of the treatment and care process in patients.

The sample of the study consisted of seven students determined by using homogeneous sampling method which is one of the purposeful sampling methods. Necessary permits were obtained to carry out the study. “Student Information Form” and “Semi-Structured Interview Form” were used to collect the data. Interviews lasted an average of 45 minutes. The audio recordings of the interviews were analyzed by a researchers and content analysis was conducted by the researcher in order to reach the concepts and relations that could explain the data obtained. Results: Of the seven nursing first year students, four were female and three were male and the mean age was 20.57. According to the content analysis of the data, four themes that “experiences related to patient”, “experiences related to nursing interventions”, “experiences related to life” and “experiences related to perception of nursing”. In the direction of themes, 11 sub-themes were determined that “fear”, “hopelessness”, “despair”, “anger”, “difficulty”, “suffering”, “value of time”, “instil hope”, “meaning of professional autonomy”, “nurse key role”, “human dignity protector”. Conclusion: It was revealed that nursing students experienced their first clinical experiences in the palliative care, the experiences they experienced in the concept of the patient and their professions, and the diminishing depth related to the holistic and humanistic aspect of nursing.
