Özkıyım tüm dünyada çocuklarda ve erişkinlerde önemli toplum sağlığı problemleri arasında yer almaktadır. Ergenlik döneminde özkıyım düşüncesi ve girişimi -özellikle de tekrarlayan özkıyım girişimi- ile ilişkili risk faktörlerinin ve erken belirleyicilerinin belirlenmesi büyük önem taşımaktadır. Bu çalışmada ilaç zehirlenmesi tanısıyla çocuk yoğun bakım servisinde yatırılarak tetkik ve tedavi olan ergen özkıyım olgularının demografik ve özkıyım ile ilişkili faktörlerin ve özellikle tekrarlayan özkıyım girişimi olgularının ilişkili faktörlerinin belirlenmesi planlandı. Çalışmaya Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Çocuk Acil Ünitesi ve Ümraniye Eğitim Araştırma Hastanesi Çocuk Acil Ünitesine ilaç zehirlenmesi tanısıyla ardışık başvuran 12-17 yaş arasında ki 80 özkıyım (12 erkek, 68 kız) olgusu dahil edildi. Tüm olguların demografik özellikleri, özkıyım için alınan ilaç içeriği ve miktarı, daha önceden kronik organik ya da psikiyatrik hastalık varlığı, ilaç kullanımı ve daha önceden özkıyım girişimi not edildi. Çalışmaların istatistik analizi için SPSS for Windows (16.0, Chicago, IL) paket programı kullanıldı. p değerinin <0,05 olması istatistiksel olarak anlamlı kabul edildi. Çalışmaya dahil edilen 80 olgunun medyan yaşı 16 yaş idi ve olguların %86,2’si halen okula devam etmekteydi. Hastaların aldığı ilaç sayıları değerlendirildiğinde, olguların %46,2’sinin 20’den fazla ilaç aldığı öğrenildi. Hastaların %47,5’inde birden fazla grup ilaç (2-6 farklı grup ilaç) alım öyküsü vardı. Hastaların almış olduğu ilaçların dağılımında %24,8’i analjezik/antipiretik ilaçlar, %20,3’ü antidepresan ilaçlar, %17,8’i kombine anti-gribal ilaçlar, %9,5’i antibiyotik ilaçlar aldığı öğrenildi. Hastaların özkıyım için aldıkları ilaçların %46,2’si kendisine ait, %22,5’i annesine ait, %10’u ise aynı evde yaşayan büyükanne-dedeye ait ilaçlar ile özkıyım girişiminde bulunduğu öğrenildi. Olguların %30,4’ünde daha önceden psikiyatrik bir tanı olup, ilaç kullanmakta idi. Olguların %26,2’sinde daha önceden özkıyım girişimi öyküsü mevcut idi (2-6 kez). Tekrarlayan özkıyım girişiminde bulunan olguların, ilk kez özkıyım girişiminde bulunanlara göre daha çok kendi ilaçları ile özkıyım girişiminde bulunduğu (p<0,05), daha çok psikiyatrik ilaç alımı öyküsünün olduğu (p<0,05), okula devam etmeyen olgularda daha sık olduğu saptandı (p<0,01). Çalışmamızda yeniden özkıyım girişimi olan olgularda cinsiyet farkı saptanmazken, tekrarlayan özkıyım girişiminde bulunan olguların kendi ilaçları ile özkıyım girişiminde bulunma oranlarının yüksek olduğu, psikiyatrik ilaç kullanım öyküsünün ve dolayısı ile psikiyatrik hastalık tanısı alma sıklığının daha fazla olduğu ve bu olgularda okul terk durumunun daha yüksek olduğunu saptadık. Bu çocukların evde daha çok kendilerine ait ilaçlar ile yeniden özkıyım girişiminde bulunmaları, ailelere bu aşamada büyük görev düştüğünün altını çizmektedir ve başta ilaç güvenliği olmak üzere ailenin ihmali de önemli bir rol oynamaktadır. Bu çocukların kullanmış olduğu ilaçların ve diğer ilaçların ev içinde saklanması ve güvenliğinin sağlanması ebeveynlerce sağlanmalıdır ve özkıyım ile ilişkili ilaçlara erişim konusunda da düzenlemelere gereksinim duyulmaktadır.
It is one of the major health problems in children and adults around the world. It is of great importance to identify the risk factors and early determiners associated with the thought and initiative of self-suffering in adolescence – especially the repeated self-suffering initiative. In this study, the diagnosis of drug poisoning was planned to identify the factors associated with the demographic and the symptoms of adolescent ejaculation, which were inspected and treated by the child’s intensive care service, and the factors associated with the demographic and the symptoms of the repeated ejaculation initiative. Suicide is one of the important public health problems in children and adults worldwide. It is of utmost importance to identify risk factors and early determinants related to suicidal ideation and attempts, especially recurrent suicide attempts, during adolescence. In this study, it was planned to determine factors related to demographic and suicide related factors, especially recurrent suicide attempt, among adolescents, which admitted to the pediatric intensive care unit due to drug poisoning. We enrolled 80 consecutive suicide cases (12 boys, 68 girls) aged between 12-17 years who were admitted to Eskisehir Osmangazi University Faculty of Medicine, Pediatric Emergency Unit and Umraniye Educational Research Hospital Pediatric Emergency Unit, due to suicide related with drug poisoning. The demographic characteristics of all cases, drug content and amount taken for suicide, the presence of previously chronic or psychiatric disease, previous routine drug use, and prior suicide attempt history were noted. SPSS for Windows (16.0, Chicago, IL) package program was used for statistical analysis. A value of p <0.05 was considered statistically significant. The median age of the 80 patients was 16 years and 86.2% of the cases were still continuing their education. When the numbers of drugs taken by the patients were evaluated, 46.2% of the cases received more than 20 drugs. 47.5% of patients had more than one group medication (2-6 different group medications). In the distribution of the medicines taken by the patients, 24.8% were analgesic / antipyretic drugs, 20.3% antidepressant drugs, 17.8% combined common-cold drugs and 9.5% antibiotic drugs. 46.2% of the medicines taken by the patients for suicide attempt have been prescribed for index case, 22.5% have been prescribed for patient’s mother and 10% have been prescribed for household members . In 30.4% of the cases, have previous psychiatric diagnosis and have been used drugs fort his diagnosis. 26.2% of the cases had a history for recuurent suicide attempt (2-6 times). Patients with recurrent suicide attempts were more likely to have suicide attempt with their own drugs (p <0.05), more frequent psychiatric drug use (p <0.05) and more frequently abandonned from school (p<0.01) than those who attempted suicide for the first time. In our study, we found that there are no gender difference for recurrent suicide attempt, and suicide with their own medications, previous presence of psychiatric disease and drug use, and do not continue eductaion are higher in adolescents with recurrent suicide attempts. The fact that these children attempt to re-suicide more often with their own medicines underlines the fact that the family members plays key role, and drug safety concerns in home is an important neglect. Parents should be provided with the safekeeping of medicines and other medicines used by these children in the home, and there is a need to regulate access to medicines related to suicide.
