Abstract:

Purpose: Hemiartroplasty is the most commonly used therapy in the treatment of femural neck fractures in patients aged ≥60. In this study, the functional results of unipolar and bipolar endoprotheses used in hemyartroplasty in elderly patients with femur neck fractures were compared. Method: In this study, a total of 63 patients over the age of 60 were diagnosed with hemyartroplasty due to femur neck fractures between 2004-2019. 36 patients were unipolar and 27 were bipolar endoprothesis. All patients were monitored for at least 12 months. The breaks were assessed according to the classification of Pauwels. The demographic and clinical characteristics of the patients were recorded. The postoperative column function was assessed by Harris column score (HHS). The average age of the group implanted with Unipolar endoprothesis was 74.1±7.3 years, and the average age of the group implanted with bipolar endoprothesis was 72.9±8.3 years. There was no significant difference between the groups in terms of age, gender and broken localization. According to the Pauwels classification, there was a significant difference between the two endoprothesis groups in terms of broken types (p<0,01). Tracking time, dislocation and wound place infections were similar among the groups. The average score of HHS’s toping, shoe wear, mobility and subtitles on a public transport vehicle was significantly higher in patients with bipolar arthroplasty (secondly; p=0,014, p=0,020, p=0,026, and p=0,03). However, the total HHS score was similar between groups (p=0,728). Results: Our study results show that bipolar artroplastine gives better results in patients where mobilization can be better predicted according to HSS subtitles. However, unipolar arthroplasty is a lower-cost treatment method in elderly patients with femur neck fractures.
