Amaç: Araştırmamızın amacı ağır preeklampsi, eklampsi ve HELLP sendromu tanılı hastaların maternal ve perinatal morbidite ve mortalite oranlarının saptanması ve literatür verileriyle karşılaştırılmasıdır. Gereç ve Yöntem:İki yıllık sürede ağır preeklampsi, eklampsi ve Hellp sendromu tanısı ile izlenen ve elektronik veri bilgi sistemine kaydedilen hastaların verileri retrospektif olarak incelendi. Bulgular: Çalışma periyodu boyunca toplam 33719 doğum gerçekleşti. Hastanemizde ağır preeklampsi oranı %1.2 (n =407), Eklampsi oranı %0.12 (n =41), HELLP sendromu oranı %0.2 (n =65) olarak tespit edildi. Çalışmamızda maternal yaş, gravide ve parite HELLP sendromu grubunda, ağır preeklampsi ve eklampsi grubuna göre daha yüksek saptandı. Sezaryen doğum oranı eklampsi grubunda, preeklampsi ve HELLP sendromu grubuna göre daha yüksek saptandı (sırasıyla %80.5, %75.4, %68.8) (p =0.005). Maternal morbidite oranı HELLP sendromu ve eklampsi gruplarında, preeklampsi grubuna göre daha fazla saptandı (sırasıyla %13.8, %12.2, %4) (p <0.0001). Akut böbrek yetmezliği (ABY) en sık maternal morbidite nedeni olarak gözlendi. En sık ABY, HELLP sendromu grubunda saptandı (%10.8). Neonatal morbidite ve perinatal mortalite oranları açısından gruplar arasında anlamlı fark saptanmadı. Sonuç: Bizim çalışmamız maternal morbidite açısından HELLP sendromu ve eklampsinin ağır preeklampsiye göre daha yüksek oranda kötü sonuçlara yol açtığını ortaya koymaktadır. Erken tanı ve zamanında uygun hızlı tedavi ile sonuçları iyileştirmek mümkündür.
Purpose: The purpose of our research is to identify and compare the rates of maternal and perinatal morbidity and mortality of patients with severe preeclampsia, eklampsia and HELLP syndrome with literary data. In the course of two years, the data of patients monitored with the diagnosis of severe preeclampsia, eklampsia and Hellp syndrome and recorded in the electronic data information system were retrospective examined. Results: A total of 33719 births occurred during the working period. In our hospital, the rate of severe preeclampsia was 1.2% (n = 407), the rate of eclampsia was 0.12% (n = 41), the rate of HELLP syndrome was 0.2% (n = 65). In our study, maternal age, pregnancy and parity HELLP syndrome group were found higher than the severe preeclampsia and eklampsia group. Caesar birth rates were higher in the eklampsia group, compared to the preeclampsia and HELLP syndrome group (relevantly 80.5%, 75.4%, 68.8) (p = 0.005). The maternal morbidity rate was more detected in HELLP syndrome and eklampsia groups compared to the preeclampsia group (13.8 percent, 12.2 percent, 4 percent) (p <0.0001). Acute kidney failure (ABY) was most often seen as the cause of maternal morbidity. The most common ABY was detected in the HELLP syndrome group (10.8%). There is no significant difference between neonatal morbidity and perinatal mortality rates. The result: Our study shows that HELLP syndrome and joints in terms of maternal morbidity lead to a higher rate of bad results than severe preeclampsia. It is possible to improve the results with early diagnosis and appropriate rapid treatment in time.
