SAĞLIK SEKTÖRÜNDE KAMU ÖZEL ORTAKLIĞI: ŞEHİR HASTANELERİ ÜZERİNE BİR DEĞERLENDİRME

Abstract:

Kamu kurumlarında yaşanan gelir gider dengesizlikleri, kaliteyi artırma gibi nedenlerle pek çok ülkede kamu özel ortaklığı kullanılmaktadır. Türkiye’de de neoliberal politikaların etkisiyle sağlık hizmetlerinin sunumunda kamu özel ortaklığı projeleri uygulanmaya başlamıştır. Kamu ve özel sektörden oluşan paydaşlar, bu ortaklıklara ait tesislerin inşası ve işletilmesi sürecinde birlikte çalışmaktadır. Şehir hastaneleri ya da sağlık kampüsleri olarak adlandırılan ve bir kısmı hizmete açılan bu tesislerin, kamu özel ortaklığı ile yapılmasına ilişkin eleştiriler tartışılmaya başlanmıştır. Bu doğrultuda çalışmada, şehir hastanelerinin kamu özel ortaklığı ile yapılmasına ilişkin eleştirilerin değerlendirilmesi amaçlanmıştır. Derleme olarak hazırlanan çalışmada şehir hastanelerine yönelik eleştiriler bir arada verilmeye çalışılmıştır. Kamu özel ortaklıkları ve şehir hastaneleri ile ilgili literatürle birlikte finansal, hukuki ve ekonomik eleştirilerin yanı sıra riskler ve eşitsizlikler açısından yapılan eleştiriler de incelenmiştir. Sonuç olarak, finansal kurumların bu ortaklıklara fon sağlamadaki isteksizlikleri, devletin maddi kayıp yaşayacağı düşüncesi, sözleşmelerin uzun süreli olması, bu projelerle yapılan tesislerde oluşan fiziki sıkıntılar, ulaşım sorunları ve bölgesel dağılımdaki eşitsizlikler, denetimle ilgili belirsizlikler kamu özel ortaklığına yöneltilen eleştiriler olarak görülmektedir.

Public Special Partnership in the Health Sector: A Assessment of City Hospitals

In public institutions, public private partnerships are used in many countries for reasons such as incompatibility in public institutions, quality improvement. In Turkey, with the influence of neoliberal policies, public-private partnership projects have begun to be implemented in the presentation of health services. Public and private sector stakeholders work together in the construction and operation of the facilities belonging to these partnerships. The criticism of the public private partnership of these facilities, which are called city hospitals or health campus and some are open to service, has begun to be discussed. In this direction, the study aims to evaluate the criticism of the public private partnership of the city hospitals. The study, which was prepared as a collection, sought to criticize the city hospitals. In addition to public private partnerships and literature related to urban hospitals, financial, legal and economic criticisms as well as criticisms made in terms of risks and inequality have been studied. As a result, financial institutions are unwilling to provide funds to these partnerships, the idea that the government will suffer financial losses, the long-term contracts, physical difficulties in the facilities made with these projects, transport problems and inequalities in regional distribution, uncertainty about audit are seen as criticisms directed to public private partnerships.

Public private partnerships are used in many countries for reasons such as income and expense imbalances experienced in public institutions and increasing quality. Turkey also in the provision of health services under the influence of neoliberal policies began to be implemented public-private partnership projects. The public and private sector stakeholders work together in the construction and operation of the facilities of these partnerships. Criticisms about the construction of these facilities, which are called city hospitals or health campuses and some of which were put into service, with a public private partnership, have begun to be discussed. In this direction, the aim of this study is to evaluate the criticisms regarding the construction of city hospitals with a public private partnership. In the study prepared as a review, criticisms about city hospitals were tried to be given together. Along with the literature on public private partnerships and city hospitals, financial, legal and economic criticism, as well as criticism in terms of risks and inequalities, were examined. As a result, the unwillingness of financial institutions to provide funding for these partnerships, the idea that the state will experience financial losses, long term contracts, physical difficulties in the facilities built with these projects, transportation problems and inequalities in the regional distribution, uncertainties about the audit are seen as criticisms directed to the public private partnership.
