Prematüre Bebeklerde Önemli Bir Morbidite Nedeni; Nekrotizan Enterokolit: Dokuz Yıllık Çalışmanın Sonuçları

Amaç: Bu çalışmada yenidoğan yoğun bakım ünitemizin Nekrotizan Enterokolit (NEK) sıklığını, anne ve bebeğe ait risk faktorlerini ve bu faktörlerin NEK gelişimine ve mortaliteye etkilerini değerlendirmeyi amaçladık. Gereç ve Yöntemler: Ocak 2002-Ağustos 2010 tarihleri arasında yenidoğan yoğunbakım ünitemizde yatarak takip edilen ve NEK tanısı olan 332 yenidoğanın verileri retrospektif olarak incelendi. Bulgular: Yenidoğan yoğunbakım ünitemizin NEK sıklığı %3.8 idi. Olguların 5’i term, 327’si preterm olup, ortalama doğum ağırlığı 1265±338 gr (540-2490) ve ortalama gebelik haftası (GH) 30.9±2.7 hafta (20-38) idi. En önemli risk faktörleri perinatal asfiksi (PNA), ağır RDS, ventilatör tedavisi, umblikal kateter uygulaması ve aminofilin kullanmı idi. NEK’li olgularda kanıtlanmış sepsis oranı % 9 olup, gram negatif bakteriler en sık etkendi. Bir olguda Brusella tanımlandı. Trombositopeni sıklığı % 32,5 olup, ileri evrelerde anlamlı yüksekti. Bin g’ın altındaki prematürelerin semptomlarının başlama yaşı geç (14-28 gün), hastanede yatış süreleri uzun cerrahi tedavi ve mortalite oranları yüksekti. Yirmi dört hastaya cerrahi girişim yapıldı. Cerrahi mortalite 6 (% 24), toplam mortalite 37 (% 11) bulundu. Sonuç: Nekrotizan enterokolit 32 haftanın altındaki çok riskli prematürelerin önemli bir hastalığı olup, yaşam süresi arttıkça yoğunbakım ünitelerinde bu morbidite sorun olmaya devam devam edecektir.

Purpose: In this study, we aimed at assessing the frequency of our newborn intensive care unit, the risk factors for the mother and baby, and the effects of these factors on the development of the newborn and mortality. Tools and Methods: The data of 332 newborns who were traced in bed and diagnosed with NEK in our newborn intensive care unit between January 2002 and August 2010 were examined retrospective. Results: Our newborn intensive care unit’s NEK rate was 3.8%. 5 of the cases were term, 327 were preeterm, and the average birth weight was 1265±338 grams (540-2490) and the average pregnancy week (GH) was 30.9±2.7 weeks (20-38). The most important risk factors were perinatal asphyxia (PNA), severe RDS, ventilator therapy, the application of umblikal catheter and the use of aminofiline. The proven sepsis rate in NEK cases was 9%, and gram-negative bacteria were most frequently affected. In one case, Brussels was identified. The frequency of thrombocytopenia was 32.5%, and was significantly high in advanced stages. The beginning of symptoms of prematurities below a thousand g was late age (14-28 days), the hospitalization periods of long surgical treatment and mortality rates increased. Twenty-four patients were operated. The surgical mortality was 6 (24%) and the total mortality was 37 (11%) The result: Nekrotic enterocolitis is a major disease of high-risk prematurities under 32 weeks, and as the lifetime increases, this morbidity will continue to be a problem in intensive care units.
