Covid 19 Pandemisinin, Bölgemizdeki 3. Basamak Pandemi Hastanesindeki Doğum Sayısı Ve Şekline Etkisi

Amaç: Covid 19 pandemisi sırasında dünyanın bazı bölgelerindeki karantina uygulamaları, toplumsal izolasyon ve sosyal mesafe gibi önlemler hayatımızdaki pek çok tercihimizi etkilediği gibi gebelerin bu dönemde takip ve doğum için hastane seçimlerini de etkileyebilir. Çalışmamızda; pandemi dönemi ile bir önceki yıl aynı zaman aralığı karşılaştırılarak bu değişimin belirlenmesi amaçlanmıştır Metod: Çalışmamız retrospektif ve kesitsel olarak bölgemizde pandemi hastanesi olarak seçilen Karabük Üniversitesi Eğitim ve Araştırma Hastanesinde yapıldı. Çalışmamıza; Türkiye’de ilk vakanın görüldüğü ve önlemlerin hızla alındığı tarih olan 11 Mart ile Türkiye’deki yeni vakaların plato çizmeye başladığı 25 nisan 2020 tarihlari arasındaki Karabük Eğitim Araştırma Hastanesinde doğum yapan tüm gebeler (pandemi grubu) ile doğumun mevsimsel farklılıklarını ortadan kaldırmak amaçlı bir yıl önceki aynı dönemde yani 11 mart-25 Nisan 2019 tarihleri arasında hastanemizde doğum yapan gebeler (kontrol grubu) alındı. Hastaların ortalama yaşları, doğum şekilleri, doğum yaptıkları gebelik haftası ve doğum tarihleri ile ilgili bilgiler için hastane istatistik verileri kullanıldı. Bulgular: Pandemi dönemindeki doğum sayıları daha düşüktü (p=0.000). Normal vajinal doğum sayısı pandemi döneminde azalmıştı (p=0.000). Primer sezaryen oranları ve eski sezaryen oranları arasında fark yoktu. (p=0.34, p=0.45). Her iki gruptaki gebelerin yaş ortalamaları benzerdi (p=0.32), gebelik haftaları benzerdi (p=0,46). Sonuç: Pandemi döneminde, 3. Basamak üniversite hastanesi kadın hastalıkları ve doğum bölümünde düzenli takibe gelen gebelerin doğum sayılarında azalma mevcuttu. Bu azalmanın nedeni hastaların; covid 19 ile enfekte hastaların olmadığı diğer özel sağlık kuruluşunu tercih etmeleridir. Çalışmamızın benzer olaylarda hastaların gösterdiği sağlık tesislerinin seçimi ve doğum şekillerinin değişimi açısından ileriki çalışmalar için referans olabileceği görülmektedir.

Objective: During the Covid 19 pandemic, measures such as quarantine practices, social isolation and social distance in some parts of the world may affect many choices in our lives, as well as the choice of pregnant women for follow-up and delivery during this period. In our study; It is aimed to determine this change by comparing the pandemic period with the same time interval of the previous year. Method: Our study was conducted retrospectively and cross-sectionally in Karabuk University Training and Research Hospital, which was selected as a pandemic hospital in our region. Our study; all pregnant women (pandemic group) were included in our study who gave birth in Karabük Training and Research Hospital between 11 March and April 25, 2020 which the first Covid 19 case was seen and the new cases were starting to plateau boots in Turkey. Also as a control group, all pregnant women were included in our study who gave birth between 11 March and April 25, 2019 with the purpose of a year earlier in the same period. Hospital statistics data were used for information about the average age of the patients, their birth patterns, the week of gestation and birth dates. Results: The number of births during the pandemic period was lower (p = 0,000). The normal number of vaginal births decreased during the pandemic period (p = 0,000). There was no difference between primary cesarean rates and previous cesarean rates. (p = 0.34, p = 0.45) The average ages of the pregnant women in both groups were similar (p = 0.32), the gestational weeks were similar (p = 0.46). Conclusion: During the pandemic period, there was a decrease in the number of births of pregnant women who came under regular follow-up in the Department of Obstetrics and Gynecology at the 3rd Step university hospital. The reason for this decrease is that the patients; They prefer other private health institutions without covid 19 infected patients. It is seen that our study may be a reference for future studies in terms of the selection of health facilities shown by patients and the change of birth methods in similar events.
