Amaç: Çalışmanın amacı, 1040 yatak kapasiteli Malatya Eğitim ve Araştırma Hastanesi ameliyathanelerinde verimlilik ve kalite uygulamaları süreç değerlendirme ölçütlerinin ameliyat masalarının etkin ve verimli kullanımına etkisini analiz etmektir. Aynı zamanda çalışmamız; hasta bekleme sürelerinin azaltılması, hastane kaynaklarının; zaman, emek, maliyet açısından en uygun ve verimli şekilde kullanılmasını sağlamak için yol gösterici olacaktır. Gereç ve Yöntemler: Çalışmada, Kliniklerin ameliyat masası kullanım oranları 2018 yılı 01.01.2018-31.12.2018 tarihleri arasındaki çalışma gün ve saatine ait veriler kullanılarak hesaplanmıştır. Çalışmada kullanılan hesaplama yöntemlerinin bir kısmı Sağlıkta Kalite Standartları Ameliyat Masası Kullanım Oranları indikatör kartındaki formüller baz alınarak, her yıl revize edilen 2018 yılı Verimlilik Yerinde Değerlendirme Rehberi kapsamında ameliyat masalarının etkin ve verimli kullanımını değerlendirmeye yönelik Yönetici takip ekranları ile ilgili standartlar da gözden geçirilmiş çalışmamızın dönemsel analizinin yapılmasına referans olmuştur. Bulgular: Kliniklerin ameliyat masası kullanım oranları hastane otomasyon sistemi üzerinden çekilen 2018 yılı 01.01.2018-31.12.2018 tarihleri arasındaki 3’er aylık olmak üzere 4 dönem şeklinde çalışma gün ve saatine ait veriler kullanılarak gerçekleştirilmiştir.Verimlilik ve Kalite Uygulamaları Daire Başkanlığı koordinasyonunda yapılan dış denetimler ve değerlendirme ölçütleri içerisinde yer alan Yönetici Takip Ekranları ile yapılan değerlendirmelere göre Etkililik, Etkinlik, Verimlilik Zamanlılık, Maliyet vs. gibi uygulamalara ilişkin farkındalığı, bilgi ve benimsenme düzeyi gibi konularda sürekli iyileştirmeler yapılmakta ve düzeltici önleyici faaliyetler başlatıldığı gözlenmektedir. Sonuç: Bakanlığımızın strateji ve hedefleri doğrultusunda sağlık hizmetinin kalite ve verimliliğinin artırılması, sürekliliğinin sağlanması, sağlık hizmeti sunumundan duyulan memnuniyetin üst düzeye çıkarılması ve etkin sağlık yönetimi için kaynak kullanımının ve hizmet sunumunun etkinlik ve verimliliğinin değerlendirilmesi amacıyla yılda bir kez Verimlilik Yerinde Değerlendirme dış denetimleri yapılmaktadır. 2018 yılında revize edilen standartlara ek olarak Sağlık Bilgi Yönetim Sisteminde (SBYS) Yönetici Takip Ekranlarının oluşturulması ile ilgili Bakanlığımız Kamu Hastaneleri Genel Müdürlüğü’nce 19.06.2018 tarihinde bir yazı yayınlanmıştır. Bu yazı ile birlikte hem sağlık tesisi üst yönetiminin tüm hizmet süreçlerini yakın takip etmesi, sorunu kaynağında çözmesi ve yapılacal planlamalara rehber olması niteliğiyle kurumların farkındalığı açısından oldukça fazla önem arzeder hale gelmiştir. Bu bağlamda hasta ameliyat randevuları SBYS üzerinden verilmesi zorunlu hale gelmiş, ertelenen ve iptal edilen ameliyat sayılarının takibi yılın ikinci yarısından itibaren uygulanmaya başlanmıştır. Hastaneler tarafından YTE’nin oluşturulması ile birlikte Ameliyat Masalarının kullanım oranları, ameliyatlar arasındaki bekleme sürelerinin takibi ile birlikte branşlara ayrılan ameliyat masası kullanım oranları daha efektif olarak takip edilebilmiştir. Tablo 3 incelendiğinde YTE kurulmadan önce ameliyat masası kullanım oranı dönemsel olarak %100’ü geçen oranlara sahipken YTE kurulduktan sonra (3. Dönem itibariyle) ameliyat masası kullanım oranlarının %100’ü geçmediği belirlenmiştir.

Some of the calculation methods used in the study are based on the formulas in the Health Quality Standards Operating Table Usage Rates indicator card and within the scope of the 2018 Efficiency On-Site Assessment Guide revised annually, the standards related to the manager follow-up screens for evaluating the effective and efficient use of the operating tables are reviewed. and reference. Results: The operating table usage rates of the clinics were performed by using the data of the working day and time in the form of 4 periods between the period of 01.01.2018 and 31.12.2018. Continuous improvements are made on issues such as awareness, knowledge and level of awareness about applications such as Effectiveness, Efficiency, Timeliness, Cost etc. according to the evaluations made with external monitoring and evaluation criteria included under the coordination of the Department of Productivity and Quality Practices. Activities are being observed. In order to increase the quality and efficiency of health services, to ensure continuity, to increase the satisfaction of health services delivery and to evaluate the efficiency and efficiency of service delivery and efficiency of efficient health management, external audits are conducted once a year. In addition to the standards revised in 2018, a letter was published by the General Directorate of Public Hospitals of the Ministry of Health on the establishment of the Manager Monitoring Screens in the Health Information Management System (SBYS) on 19.06.2018. has become very important in terms of the awareness of the institutions as it solves the problem at its source and provides guidance for constructive planning. In this context, it has become mandatory to make patient surgery appointments via SBYS, and follow-up of the number of postponed and cancelled surgeries started to be implemented as of the second half of the year. With the establishment of YTE by the hospitals, the usage rates of the operating tables and the use of the operating tables allocated to the branches with the follow up of the waiting times between the operations could be monitored more effectively. When Table 3 is examined, it was determined that the rate of operating table use before the establishment of YTE had a periodicity exceeding 100%, while after the establishment of YTE (as of the 3rd period), it was determined that the operating table use rate was not exceeding 100%.
