Amaç: Yaşlı hastalar, genç hastalara göre spinal anestezi altında hemodinamik instabiliteye daha eğilimlidirler. Bu çalışmanın amacı, spinal anestezi altında transüretral prostat rezeksiyon (TUR-P) ameliyatı yapılacak yaşlı hastalarda 100 μg morfine ilave edilen üç farklı izobarik bupivakain dozunun (5 mg, 7.5 mg ve 10 mg) subaraknoid blok özellikleri, postoperatif analjezi yoğunluğu ve yan etkilerini karşılaştırmaktır. Gereç ve Yöntem: TUR-P uygulanan hastalar intratekal verilen izobarik bupivakain dozuna göre 5 mg (Grup Bup5), 7.5 mg (Grup Bup7.5) ve 10 mg (Grup Bup10) her grupta n =20 olacak şekilde üç gruba ayrıldı. Tüm gruplarda, izobarik bupivakaine 100 ug morfin ilave edildi. Duyusal bloğun başlangıç ve sonlanma zamanları, motor bloğun derecesi ve maksimum zamanı, yan etkiler ve postoperatif analjezinin kalitesi kaydedildi ve gruplar arasında karşılaştırıldı. Bulgular: Maksimum duyusal blokaj seviyeleri Grup Bup5'te T10, Grup Bup7.5'te T8 ve Grup Bup10'da T6 idi. Duyusal bloğun iki segment regresyon süresi, mobilizasyon süresi ve postoperatif ilk analjezik gereksinimine kadar geçen süre Grup Bup5'te en kısa idi (p <0.001). Grup Bup10'da hemodinamik instabilite insidansı en yüksekti (p <0.001). Sonuç: Yaşlı TUR-P hastalarında 7.5 mg izobarik bupivakain + 100 μg morfin kombinasyonu, 5 veya 10 mg izobarik bupivakaine ilave benzer doz morfin uygulamasına kıyasla majör yan etkilere neden olmadan iyi kalitede spinal anestezi ve postoperatif analjezi sağlayabilir.
Objective: Elderly patients are more likely to hemodynamic instability under spinal anesthesia than young patients. The aim of this study is to compare the subaraknoid bloc characteristics of three different isobarik bupivakain doses (5 mg, 7.5 mg and 10 mg) which are added to 100 μg morphine in elderly patients under spinal anesthesia, the intensity of postoperative analgesia and side effects. Patients with TUR-P were divided into three groups according to the intracal dose of 5 mg (Group Bup5), 7.5 mg (Group Bup7.5) and 10 mg (Group Bup10) in order to be n = 20 in each group. In all groups, 100 ug of morphine was added to isobarik bupivakaine. The start and end times of the emotional block, the degree and maximum time of the motor block, side effects and the quality of postoperative analgesia were recorded and compared between the groups. The maximum sensory blockage levels were T10 in Group Bup5, T8 in Group Bup7.5 and T6 in Group Bup10. The two segment regression time of the emotional block, the mobilization time and the time passed until the postoperative first analgesic requirement were the shortest in Group Bup5 (p <0.001). The highest incidence of hemodynamic instability in the Bup10 group was (p <0.001). The result: In older TUR-P patients, a combination of 7.5 mg isobary bupivakain + 100 μg of morphine, 5 or 10 mg isobary bupivakaine, a supplementary dosage of similar morphine can provide high-quality spinal anesthesia and postoperative analgesia without causing major side effects.
