AMAÇ: Bu çalışmada, hastaneye yatış gerektiren dirençli epistaksis (DE) olgularında mevsimsel dağılım, komorbiditeler ve tedavi süreci incelendi ve DE etiyolojisi ve etiyolojiye göre uygun tedavi seçenekleri değerlendirildi. YÖNTEMLER: Ocak 2016 - Aralık 2019 tarihleri arasında Kulak, Burun, Boğaz Bölümü tarafından epistaksis tanısı konulan toplam 11370 hasta (7114 erkek, 4256 kadın; ort. yaş: 56.7±19.9 yıl; dağılım, 3-95 yıl), hastanemizin veri tabanı üzerinden geriye dönük olarak tarandı. Hastalar hastaneye yatış durumlarına göre DE ve basit epistaksis (BE) olmak üzere iki gruba ayrıldı ve başvuruların mevsimsel dağılımına bakıldı. Dirençli epistaksis grubunda yaş, antikoagülan kullanımı, hipertansiyon, yatış süresi, uygulanan tedavi yöntemleri ve kan transfüzyon parametreleri incelendi ve birbirleriyle olan ilişkileri değerlendirildi. BULGULAR: Mevsimler arası DE oranlarında istatistiksel olarak anlamlı bir fark saptanmadı (p=0.185). Hipertansiyon hastalarında konservatif yöntemler ile kanama kontrolü sağlama oranı (%57.4, p>0.001), istatistiksel olarak anlamlı derecede yüksek ve cerrahi müdahale ile kanama kontrolü sağlama oranı (%10.5, p=0.01), istatistiksel olarak anlamlı derecede düşük bulundu. Yaşlı hastalarda (>51 yaş) anterior ve posterior tampon uygulaması (%76.9, p=0.002) ile kanama kontrolü sağlama oranı ve genç (<51 yaş) hastalarda cerrahi müdahale (%26.3, p=0.017) ile kanama kontrolü sağlama oranı istatistiksel olarak anlamlı derecede yüksek bulundu. SONUÇ: Mevsimsel farklılıklar epistaksis gelişiminde etkili olabilse de, sonuçlarımız mevsimsel farklılıkların DE gelişimini ve tedavi sürecini etkilemediğini göstermektedir. Tüm bu bilgiler ışığında, DE olgularında tedavi yöntemini seçerken etiyolojik faktörlerin de göz önünde bulundurulması gerekmektedir. Goal: In this study, the seasonal distribution, comorbidities and treatment process in resistant epistaksis (DE) cases requiring hospitalization were studied and appropriate treatment options were evaluated according to the ethology and the ethology of the DE. Methods: A total of 11370 patients diagnosed with epistaksis by the Ear, Nose, Gorge Department between January 2016 and December 2019 (7114 men, 4256 women; ort. There is no statistically meaningful difference in seasonal DE ratio (p=0.185). In patients with hypertension, the rate of blood control by conservative methods (57.4, p>0.001), statistically significantly high and the rate of blood control by surgery (10.5, p=0.01), statistically significantly low was found. In the elderly patients (>51 years of age) with the application of the anterior and posterior tampon (%76.9, p=0.002) the rate of blood control and in the young patients (<51 years of age) surgery (%26.3, p=0. Results: Although seasonal differences can be effective in the development of epistaksis, our results show that seasonal differences do not affect the development of DE and the process of treatment. In the light of all this information, the method of treatment in cases of AD should also be taken into account by the etiological factors.
