Amaç:Donuk omuz olgularında, yüksek güçte ağrı sınırında ultrasonun (YGAS-US) omuz ağrısı, eklem hareket açıklığı (EHA) ve üst ekstremite fonksiyonları üzerine etkisinin konvansiyonel ultrasona (K-US) üstünlüğünü araştırmaktı. Yöntem:Donuk omuz tanısı olan 20 gönüllü olgu rastgele iki gruba ayrıldı. Her iki grubun tedavi süresi 14 seans ve standart tedavi programları: TENS, Cold Pack, NMES ve egzersiz eğitimi olarak belirlendi. Standart tedavi programlarına ek olarak çalışma grubuna YGAS-US, kontrol grubuna ise K-US, 5’er dakika süreyle uygulandı. Tüm hastalar tedavi öncesinde, 7. seansta ve 14. seansta olmak üzere 3 kez değerlendirildi. Omuz ağrısı " Görsel Analog Skalası (GAS)", eklem hareket açıklığı (EHA), "Gonyometrik Ölçüm", kas kuvveti "Manuel Kas Testi", fonksiyonel durum "Kol, Omuz, El Yaralanması Anketi (DASH)" ile değerlendirildi. Grup içi tedavi öncesi, 7. seans ve 14. seans değerlendirmelerinin karşılaştırılması Friedman testi ile, gruplar arası karşılaştırmalar ise Mann-Whitney-U ve iki yönlü ANOVA testi ile yapıldı. Sonuçlar:Her iki grupta da EHA, GAS ve DASH puanlarında tedavi sonrası anlamlı düzelmeler görüldü (p≤0,005). Tedavi öncesi ve sonrası elde edilen bu puanların farkı alınarak hesaplanan, grup içi değişimleri karşılaştırıldığında, gruplar arasında anlamlı fark olmadığı görüldü (p>0.05). Tartışma:Sonuç olarak donuk omuz tedavisinde K-US ve YGAS-US uygulamaları değerlendirilen değişkenler üzerinde etkin bulunmuştur ancak YGAS-US’un K-US’a üstünlüğü gösterilememiştir.
Purpose:In spinal shoulder cases, high-power pain limit ultrasound (YGAS-US) was to study the prevalence of conventional ultrasound (K-US) of its effect on shoulder pain, joint movement openness (EHA) and upper end functions. Method: 20 volunteer facts with a thicken shoulder diagnosis were randomly divided into two groups. The treatment period of both groups is defined as 14 sessions and standard treatment programs: TENS, Cold Pack, NMES and exercise training. In addition to the standard treatment programs, the study group was applied to YGAS-US and the control group to K-US for 5 minutes. All patients were evaluated 3 times before treatment, 7th session and 14th session. The shoulder pain was assessed by the "Visual Analog Scale (GAS)", joint movement openness (EHA), "Gonyometric Measurement", muscle strength "Manuel Muscle Test", functional status "Kol, Shoulder, Hand Injury Survey (DASH)" The comparison of the pre-group, 7th and 14th session assessments was made with the Friedman test, and the intergroup comparisons were made with the Mann-Whitney-U and the two-way ANOVA test. Results: Both groups saw significant improvements after treatment in EHA, GAS and DASH points (p≤0,005). Compared to the group changes, which were calculated by the difference between these points obtained before and after treatment, there was no significant difference between the groups (p>0.05). Discussions:As a result, the K-US and YGAS-US applications in the treatment of spinal shoulders have been effective on the evaluated variables; however, YGAS-US’s superiority over the K-US has not been shown.
