PRİMİPAR ANNELERİN SÜTÜNDEKİ YAPISAL DEĞİŞİKLİKLER İLE POSTNATAL EMZİRME ÖZ YETERLİLİK İLİŞKİSİNİN İNCELENMESİ

Amaç: Primipar annelerin sütündeki yapısal değişiklikler ile postnatal emzirme öz yeterlilik ilişkisini incelenmektir. Gereç ve Yöntem: Prospektif, tanımlayıcı ve ilişki arayıcı özellikteki araştırma, Aralık 2016-Ağustos 2017 tarihleri arasında, İstanbul’da bir Eğitim ve Araştırma Hastanesi’nde yürütüldü. Araştırmanın örneklemini 320 anne oluşturdu. Veriler Postnatal “Emzirme Öz Yeterlilik Ölçeği-Kısa Şekli” ve araştırmacılar tarafından hazırlanan Tanıtıcı Bilgi Formu kullanılarak ilk 24 saatte yüz yüze görüşme yöntemi ile ve 15. günde telefon ile görüşme yöntemi ile elde edildi. Bulgular ve Sonuç:  Araştırmaya katılan annelerin yaş ortalamasının 23,99±3.85 olduğu, %52.2’sinin orta eğitim mezunu olduğu ve %65’inin çalışmadığı belirlendi. Annelerin yaşı, eğitim ve çalışma durumuna göre postnatal emzirme öz yeterlilik ölçeği 24. saat ve 15. gün puan ortalamaları arasında istatistiksel olarak anlamlı bir fark olmadığı görüldü (p>0.05).  Annelerin bebeği ilk kucağa alma zamanı, doğumdan sonra ilk emzirme, ilk 24 saatte emzirme sıklığı, emzirmeye tanık olma durumu ile 24. saat ve 15. günde Postanal Emzirme Öz Yeterlilik Ölçeği puanları arasında istatistiksel olarak anlamlı fark olduğu belirlendi (p>0.05). Vajinal ve sezaryen doğum yapan annelerin, doğumdan sonra ilk 24. saatte yapılan ölçümlerde emzirme öz yeterlilik düzeyinin 15. günde yapılan ölçümlere göre düşük olduğu, istatistiksel olarak anlamlı fark olduğu saptandı (p<0,000). Bebeğini kucağına erken alan, doğumdan sonra ilk bir saatte emziren, ilk 24 saatte sık emziren ve bebeğine emzik/biberon vermeyen annelerin emzirme öz yeterlilik düzeyi yüksekti. Vajinal doğum yapan primipar annelerin emzirme öz yeterlilik düzeyi sezaryan doğum yapanlara göre daha yüksekti. Vajinal ve sezaryen doğum yapan primipar annelerin 15.gündeki emzirme öz yeterlilik düzeyi, ilk 24 saate göre anlamlı olarak yüksek idi.

Title: PRIMIPAR MOTHER'S MILK'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER'S PRIMIPAR MOTHER Tools and Methods: Prospective, identifying and relationship-seeking research was carried out in an Education and Research Hospital in Istanbul between December 2016-August 2017. The study was carried out by 320 women. The data was obtained using the Postnatal "Small Breastfeeding Qualification-Short Form" and the Indicative Information Form prepared by researchers with the first 24 hours of face-to-face conversation method and the 15th day of telephone conversation method. Results and Results: The average age of the mothers who participated in the study is 23.99±3. 85% were high school graduates, 52.2% were high school graduates and 65% were not working. According to the age, education and working conditions of the mothers, the postnatal breastfeeding self-sufficiency scale found no statistically significant difference between 24 hours and 15 days (p>0.05).  It was statistically significant difference between the time when the mothers took the baby to the first breastfeeding, the first breastfeeding after birth, the frequency of breastfeeding in the first 24 hours, the status of witness to breastfeeding in 24 hours and the Postanal Breastfeeding Qualification Score scores in the 15th day (p>0.05). In the first 24 hours after birth, the level of breastfeeding self-sufficiency was found to be low, statistically significant, compared to the measurements made in the 15th day (p<0,000). Mothers who breastfeed their baby early, breastfeed in the first hour after childbirth, breastfeed frequently in the first 24 hours and don’t breastfeed their baby/biberon were high levels of self-sufficiency. Primipar mothers who have a vaginal birth had a higher level of self-sufficiency than those who have a maternal birth. The level of breastfeeding self-sufficiency in the 15th day of the primipar mothers who were having a vaginal and cesarean birth was significantly higher than in the first 24 hours. Objective: We aim to investigate the breastfeeding self-efficacy levels of primiparous mothers by their type of delivery and breastfeeding-related characteristics. Methods: A descriptive study was conducted in a Training and Research Hospital in Istanbul between December 2016 – August 2017. The study sample included 320 mothers. The data were obtained via face-to-face interviews in the first 24 hours and via telephone interviews on the 15th day using “Breastfeeding Self-Efficacy Scale-Short Form” and the “Introductory Information Form” prepared by the researchers. Results: The mean age of the mothers who participated in the study was 23.99 ± 3.85 and 52.2% of them were middle school graduates and 65% of them did not work. The 24th-hour and 15th-day mean scores of breastfeeding self-efficacy scale did not show any statistically significant difference by their age, education and working status (p>0.05). A statistically significant difference was found between the mothers’ first time to take the baby on their lap, the first breastfeeding after birth, the frequency of breastfeeding in the first 24 hours, the status of witnessing breastfeeding and the Breastfeeding Self- Efficacy Scale scores at 24 hours and 15 days (p< 0.05). Mothers who had vaginal and caesarean births were found to have low levels of breastfeeding self-efficacy in the first 24 hours after birth compared to the measurements performed on the 15th day, and the difference was statistically significant (p <0.000). Conclusion: The perception of breastfeeding self-efficacy of primiparous mothers who delivered vaginally was higher than those delivered by cesarean section. Primipar mothers’ perception of breastfeeding self-efficacy increases two weeks after delivery compared to the first day.
