Abstract:

Purpose: Treatment of thicknesses in the elderly still involves difficulties. In our study, we aimed at evaluating the short-term clinical results of long-stemless arthroplasty in elderly patients with femur neck or intertrocanterial fractures. Tools and Methods: From January 2011 to December 2019, we collected data of patients we were treated with long-stainless thumbless arthroplasty due to thickness. 48 patients (≥65 years of age) with femur neck or intertrocanteric fractures were included in the study. Bipolar or total arthroplasty was applied to patients. All patients were followed for at least 2 years. The results were evaluated in terms of Harris' headache score (HHS), hunting categories, complications, headache pain and pre-headache pain. We compared the clinical results of patients with femur neck fractures and patients with intertrocantheric fractures. The average tracking time was 57 (34-92) months. In sixteen patients, the femur neck was broken, and in 32 patients, the intertrocanteric was broken. There was no difference between the groups in terms of demographic characteristics and comorbidities. Preoperative, Barthel's daily life activity index score was calculated at an average of 79. The average in the latest checks was HHS 77 and Koval categories 3. Two patients were reviewed due to the outbreak. Only 1 of the patients had an infection and was treated with debridman and antibiotics. Implant deficiency or periprosthetic breakdowns have not been observed in any patient. Result: With good clinical results, long-stainless stemed arthroplasty can be an option for the treatment of column fractures in elderly patients. Study with longer tracking periods will be more suitable to assess the effectiveness of these protheses.
