Abstract:

Purpose: Kienböck disease is a clinical picture characterized by avascular necrosis of lunatum, which causes progressive pain in the arm joint, loss of function and the formation of common arthrosis between the carpal bones in the long term. There is no standard treatment protocol yet. Many techniques have been defined in the treatment of Kienböck disease. In the planning of treatment, the patient’s age, the stage of the disease, the distal radio-ulnar joint relationship, the degree of arthrosis in the arm joint and the patient’s expectations are important. In this study, we aimed at presenting the clinical results of our patients treated with the radial shortening method. Tools and Methods: Between May 2008 and March 2014, 16 cases due to Kienböck's disease were treated with radial shortening osteotomy by 7 women and 9 men's volar approach. The average age of the patients was 27.2 and the left arm was affected in 6 patients and the right arm in 10 patients. The patient’s average follow-up period was 31 months. Patients before and after surgery were evaluated with joint movement widths, capability, visual pain scale, pain levels and radiological progress. Results: The pre-operative measurement of the average capability was 21.5 kg, while the final measurement was 26.5 kg and increased in all patients. The pre-operative movement width was measured at 86.5 degrees, and in the final postoperative assessment at 125.5 degrees. The VAS score, which was 7.5 before the surgery, was 2.6 during the postoperative period.
