Burdur’da Evde Sağlık Hizmeti Alan Yaşlı Hastaların Profili ve Evde Verilen Sağlık Hizmetleri

Amaç: 2011 yılında ülkemizde kamusal düzeyde evde sağlık hizmeti verilmesi fiili olarak başlatılmıştır. Çalışmada; Burdur’da evde sağlık hizmeti kullanan 65 yaş ve üzeri yaşlı hastaların profilini ve evde verilen sağlık hizmetlerini tanımlamak amaçlanmıştır. Yöntem: Tanımlayıcı tipteki bu araştırmanın evrenini, Nisan 2011 tarihinde Burdur İl Sağlık Müdürlüğü Evde Sağlık Hizmeti Koordinasyon Merkezi’ne kayıtlı 65 yaş ve üzeri 140 kişi oluşturmaktadır. Çalışmada örneklem seçilmemiş olup, evrenin tümüne ulaşılması hedeflenmiştir. Veriler anket yöntemiyle, yüz yüze görüşme tekniği kullanılarak toplanmış ve SPSS paket programı kullanılarak analizler tamamlanmıştır. Bulgular: Yaş ortalaması 79.6 olan yaşlıların %67.6’sı kadın, %53.7’sinin eşi ölmüş, %52.8’i okuryazar değil, %47.2’sinin sosyal güvencesi bulunmamakta ve %25.9’unun sağlık güvencesi yeşil karttır. Yaşlılardaçok bulunan hastalıklar hipertansiyon (%48.1) ve inmedir (%39.8). Yaşlıların kolaylıkla yapabildikleri eylemlerin başında yemek yeme gelirken (%30.6); fatura yatırma, hastaneye gitme vb. gibi aktivitelerde hemen hemen tamamen bağımlıdırlar (%99.1). Yaşlılara evde verilen tıbbi hizmetler %46.3 muayene ve ilaçla tedavi, %26.9 muayene, tetkik ve ilaçla tedavi, %14.8 enjeksiyon, sonda takma ve yara bakımıdır. Yaşlılara evde verilen sağlık hizmetlerinin %87.0’sini sadece devlet hastanesi, %3.7’sini ise aile hekimi vermiştir. Evde sağlık hizmeti verenlerin büyük kısmını (%92.6) hekimler oluşturmuştur. Sonuç: Evde sağlık hizmeti sunulan yaşlıların sosyodemografik özellikleri, hastalık örüntüleri ve bağımlılık durumu toplumdaki yaşlı populasyonundan farklıdır. Sağlık hizmet sunumuna birden fazla sağlık kurumu dâhil olmaktadır, sunulan sağlık hizmeti kapsamlı değildir ve multidisipliner ekip anlayışından yoksundur. Yaşlı nüfusun gittikçe artacağı göz önünde bulundurulduğunda, kamusal düzeyde “evde sağlık hizmeti” birimleri değil, evde sağlık hizmetlerini de içerecek biçimde “evde bakım hizmeti” birimlerinin oluşturulması gerekmektedir. This study defines the profile of elderly patients over 65 years who use home health services and home care provided in Burdur. Method: 140 people aged 65 and over, who registered in the Burdur Province Directorate of Health Service Coordination Center for Home Care in April 2011 constitute the population of this descriptive study. The sample for the study was not selected; it was targeted to include the total relevant population. A survey method was used to collect data by face-to-face interviews and the analysis was completed by using SPSS packaged software. Results: Of the elderly studied, whose mean age was 79.6, 67.6% were women, 53.7% of them were widowed, 52.8% of them were illiterate, 47.2% did not have social security and 25.9% of them had a green card as health insurance. The most common diseases in the elderly are hypertension (48.1%), and stroke (39.8%). While main action which elderly can carry out  easily is eating (30.6%) . they are almost totally dependent on help for certain activities such as bill payments, going to the hospital and so on (99.1%). Home care services provided to the elderly include medical examination and treatment (46.3%), examination, investigation and treatment (26.9%), injection, catheter insertion and wound care (14.8%). Of the home care services given to the elderly, 87.0% t was provided by the/a state hospital and 3.7% of it was provided by a family doctor. Conclusion: The sociodemographic qualities, disease patterns and addiction case of elderly to whom homecare service provided are different from the elderly population in the society. More than one health institute involve into the presentation of health service, the provided health service is not extensive and lacks multi – disciplinary team approach. Considering that the elderly population will gradually increase, it is required that not home health service units at public level but home care service which will include home health service units are to be formed. The purpose: In 2011 the public level of healthcare in our country was actually launched. The study aims to identify the profile of patients aged 65 and over who use health care at home in Burdur and the health care provided at home. Method: The universe of this identificative type of research is made up of people aged 65 and over 140 registered in the Home Health Service Coordination Center in the Burdur Provincial Health Directorate in April 2011. The study has not selected sample, and it is aimed at reaching the entire universe. The data is collected using the survey method, the face-to-face interview technique and the analysis is completed using the SPSS package program. Results: 67.6 percent of the elderly with an average age of 79.6 are women, 53.7 percent of their spouses are dead, 52.8 percent are not reading, 47 percent. 2 of them have no social insurance and 25.9% of the health insurance is green card. The most common diseases in the elderly are hypertension (48.1%) and infection (39.8%). When the elders come to eat at the beginning of the actions they can easily do (%30.6); deposit in bills, going to the hospital, etc. They are almost entirely dependent on such activities (%99.1). The medical services provided to the elderly at home are 46.3 per cent examination and drug treatment, 26.9 per cent examination, examination and drug treatment, 14.8 per cent injection, sonication and wound care. 87,0% of the health care provided to the elderly in the home was provided by the state hospital and 3.7% by the family doctor. The majority of health care providers at home (92.6) were doctors. The socio-demographic characteristics of the elderly who are provided healthcare at home, the patterns of the disease and the state of addiction differ from the elderly population in the community. The health service offer includes more than one health institution, the health service offered is not comprehensive and the multidisciplinary team is not understood. Given the growing population of the elderly, it is necessary to create "home care" units in the public level, not "home health services" units, but "home health services" units in the form that will also include home health services.
