AMAÇ: Romatiznal hafif mitral darlıklı hastalarda, pulmoner arter basıncı normalken sağ ventrikül fonksiyonları ile ilgili veri yoktur. Biz bu çalışmamızda görsel ve boyutsal olarak normal sağ ventriküle sahip hafif mitral darlığı hastalarında sağ ventrikül fonksiyonlarını doku Doppler ile değerlendirmeyi planladıkYÖNTEMLER: Çalışmaya 42 romatizmal hafif mitral darlıklı hasta ve 31 yaş ve cinsiyet eşleşmiş kontrol grubu alındı. Hastalara konvensiyonel ve doku Doppler ekokrdiyografi yapıldı. Doku Doppler parametlerinden izovolümetrik miyokardiyal akselarasyon, miyokard performans indeksi ve tepe sistolik velosite hesaplandı. BULGULAR: Her iki grupta yaş ortalaması, cinsiyet, sistolik pulmoner arter basıcı sistolik ve diyastolik kan basıncı ve kalp hızı değerleri benzerdi. Doku Doppler parametrelerinden izovolümetrik miyokardiyal akselarasyon p: 0,001 , miyokard performans indeksi p: 0,014 ve tepe sistolik velosite p: 0,001 anlamlı olarak hasta grubunda düşük saptandı. İzovolümetrik miyokardiyal akselarasyon doku Doppler parametrelerinden arasında mitral kapak alanı ileiyi korele olan paramatre olarak bulundu r: 0.59; p: 0,0001 SONUÇ: Görsel ve boyutsal olarak normal sağ ventriküle sahip romatizmal hafif mitral darlığı hastalarında doku Doopler ile değerlendirilen sağ ventrikül fonksiyonları bozulmuştur. Sağ ventrikül fonksiyonlarındaki bu bozukluk pulmoner vaskülatürdeki değişikliklerden çok sağ ventrikülün romatizmal tutulumuna bağlı olabilir.
Target: In patients with rheumatism mild mitrals, the pulmonary arterial pressure is normal, but there are no data related to the function of the right ventricle. In this study, we planned to evaluate the function of the right ventricle in patients with normal right ventricle with visual and dimensional light ventricle with tissue Doppler.MOTHES: The study was taken by 42 patients with light ventricle with a rheumatism and a 31 year old and gender-compatible control group. Patients were performed conventional and tissue Doppler ecocrdiography. The tissue Doppler parameters calculated isovolumetric myocardial acceleration, myocardial performance index and height systolic velosity. The average age, gender, sistolic pulmonary artery pressure and diastolic blood pressure and heart rate values were similar in both groups. Isovolumetric myocardial acceleration p: 0.001 from the tissue Doppler parameters, myocardial performance index p: 0.014 and height systolic velosite p: 0.001 were significantly low in the patient group. Isovolumetric myocardial acceleration tissue is found as a parameter that is correlated with the mitral cover area among the Doppler parameters r: 0.59; p: 0,0001 RESULTAT: In patients with normal right ventricle with a visual and dimensional rheumatism mild mitral depression, the right ventricle functions assessed by the Doopler tissue have been deteriorated. This disorder in the function of the right ventricle may be due to changes in the pulmonary vasculatory ventricle to the very right ventricle's rheumatism attitude.
