Purpose: The important determiner of the decision of operation in the thyroid nodes is cytology, which is considered the gold standard along with its ultrasound characteristics. It is not yet clear whether the giant nodes should be followed by cytology benignse or whether they should be operated independently of the result of cytology. The purpose of this study is to determine the rates of malignity and IAB’s wrong negative in the operated giant nodes. Materials and Methods: This study included 675 patients with a nodule of ≥ 4 cm diameter between June 2015 and April 2019. 226 of these patients were operated and the data was examined retrospectively. Results: In the histopathological study, 37 of these nodes (14.40%) were malignant, 220 (85.60%) were benign. There is no significant difference between the ultrasound characteristics and nodular diameter of the benign and malign nodules. While the two groups were similar in terms of demographic characteristics, the level of TSH was higher in malign nodes. The false negativity rate of cytology was 11.65% for giant nodules. Result: As a result, when making a surgical decision on the giant nodes, the citology and ultrasound characteristics and the presence of press symptoms or the presence of cosmetic problems should be taken into account. The rate of false negativity in cytology may be higher than small nodes, so it is necessary to follow ultrasound and repeated biopsy when necessary. These findings are not enough to recommend routine surgery.
