İnvaziv Meme Kanserinde Yaşlı ve Genç Hastaların Hormon Reseptörleri ve Moleküler Alt Tipler ile İlişkisi

Amaç: Meme kanserinde moleküler alt tiplendirme, tümör morfolojisi yerine tümör biyolojisinin temel alınmasına dayalı sınıflama sistemidir. Bu çalışmanın amacı, yaşlı nüfus sınırı olarak kabul edilen 65 yaşın, meme kanserinde moleküler düzeyde bağımsız bir risk faktörü olup olamayacağını bulmaktır. Gereç ve Yöntemler: Ankara Şehir Hastanesi Genel Cerrahi Anabilim Dalında 2012 ve 2021 yılları arasında meme kanseri tanısı ile mastektomi operasyonu yapılan 512 hasta retrospektif incelendi. Hastalar dünya sağlık örgütü (DSÖ) verilerine göre yaşlı ( ≥65 yaş) ve genç (<65 yaş) olarak iki gruba bölündü.Grupların hormon reseptörleri ve moleküler alt tipler ile ilişkisi değerlendirildi. Bulgular: Yaşlı ve genç hastaların moleküler alt gruplara göre dağılımında istatistiksel olarak anlamlı fark bulduk (p=0.038). Luminal A, ≥65 yaş hastalarda <65 yaş hastalara göre daha yüksek oranda görülmüştür ( OR=0.527, 95 %CI: 0.328-0.846, p=0.008). Sonuç: Meme kanserinde demografik bir faktörün tümör biyogenetiği ile bir ilişkisi olabileceğini bulduk. Genç ve yaşlı nüfus ayrımında kullanılan 65 yaş, meme kanseri hastalarında hormon reseptör durumu tayininde ve moleküler alt tiplerin belirlenmesinde belirleyici bir faktör olabilir.

Aim: Molecular subtypes in invasive breast cancer is a classification system based on tumor biology rather than tumor morphology. The aim of this study is to find out whether the age of 65, which is accepted as the elderly population limit, could be an independet risf factor on the molecular basis in invasive breast cancer. Materials and Methods: 512 patients who underwent mastectomy with a diagnosis of breast cancer between 2012 and 2021 at the Department of General Surgery of Ankara City Hospital were retrospectively analyzed. The patients were divided into two groups as elderly (≥65 years old) and young (<65 yeras old) according to the data of the World Health Organization (WHO). Results: The relationship of the groups with hormone receptors and molecular subtypes was evaluated. We found a statistically significant difference in the distribution of elderly and young patients according to molecular subtypes (p=0.038). Luminal A was seen at a higher rate in patients ≥65 years of age than in patients <65 years of age (OR=0.527, 95% Cl: 0.328-0.846, p=0.008) Conclusion: We found that a demographic factor in invasive breast cancer may have a relationship with tumor biogenetics. The age of 65, which is used to differentiate between young and elderly populations, may be a determining factor in determining the hormone receptor status and molecular subtypes in invasive breast cancer patients.
