Rektus kılıf hematomu, rektus kasının veya epigastrik damarlarının yırtılması sonucunda gelişen akut karın ağrısı nedenlerinden biridir. Birçok risk faktörü bulunmasına rağmen en önemli predispozan faktör antikoagülan tedavi alımıdır. İnvazif girişimler sonrasında da rektus kılıf hematomu gelişebilmektedir. Tedavisinde hastanın hemodinamisi ve hematomun tipi önemli yer tutmaktadır. Nadir rastlanan, parasentez sonrası grade III rektus kılıf hematom nedeniyle cerrahi işleme alınan olgumuzu sunmak istedik.

Rectus sheath hematoma is one of the causes of acute abdominal pain caused by rupture of rectus muscle or epigastric vessels. Although there are many risk factors, the most important predisposing factor is the use of anticoagulant treatment. Rectus sheath hematoma may occur secondary to invasive procedures. The degree of hematoma and hemodynamics of the patient is important in the treatment. We present a rare case of grade III rectus sheath hematoma after paracentesis.
