Methemoglobinemia is a clinical condition caused by the oxidation of the bivalent ferrous iron of the hemoglobin molecule to trivalent ferric iron by different oxidative stress states. Methemoglobinemia results in tissue hypoxia and may be related to congenital or acquired conditions. We aimed to discuss differential diagnosis, and treatment modalities of acute hemoglobinemia, with the considerati- on of an infant whose methemoglobinemia developed after administration of local bupivacaine prior to a circimcision and a 3 -month-old patient presented with cyanosis after prilocaine administration during liver biopsy that was done for the diagnosis of cholestasis
