Fungal poisoning has an important place in poisoning in the practical application of daily emergency service. Patients can apply to the hospital with different clinical tables depending on the type of mantra and toxin. Pantherina syndrome is a clinical picture that can be mixed with cerebrovascular diseases. Three patients from the same family, with newly developed awareness confusion, symptoms of afasia and midriasis, were requested emergency service. When the first patient was assessed, hypertensive encephalopathy was diagnosed along with the present symptoms. However, the second and third patients with similar symptoms were suspected of eating fungi with anamnesium deepening. Patients were treated in an emergency intensive care unit. Two patients recovered within 24 hours and one patient's symptoms lasted up to 72 hours. Pantherina syndrome is a clinical condition characterized by early starts, neurological symptoms such as confusion, afasia and midriasis. Treatment is usually a support therapy. Washing the stomach and active coal will be beneficial in the early period. Benzodiazepines can be used in cases of excess agitation. Pantherina syndrome is one of the diagnoses that clinics should consider in patients with an acute confound state.
