Abstract:

Amaç: Tavşanlı ilçesi halk sağlığı yetkilileri 18.06.2014 tarihinde 28’i hastanede yatarak tedavi görmüş 684 gastoenterit hastası olduğunu rapor etmişlerdir. İnceleme,  salgının nedenini saptamak, bulaş yolunu belirlemek ve kontrol önlemlerini uygulamak amacıyla yapılmıştır. Yöntem: Bu vaka-kontrol çalışmasında 07–30 Haziran tarihleri arasında R11, K52, A09 ICD-10 tanı kodlu, kusma veya ishali (≥3/gün) olan, atak hızıyüksek 5 mahallede ikamet eden, basit rastgele yöntemle seçilen 154 olası olgu ve kontrolü incelenmiştir. Kontroller, 1:1, yaş grubu eşleştirmesi yapılarak komşulardan seçilmiştir. Klinik örnekler ve şebeke suyu örnekleri incelenmiştir. Bulgular:Atak hızı %3.3 olup vakalar tüm ilçeye yayılmıştır. Başlıca şikâyetler bulantı  (%88.9), ishal (%88.2), karın ağrısı (%82.8) ve kusmadır (%71.8). Vakalar selden 6 gün sonra artmaya başlamış ve salgın eğrisinde geniş bir pik saptanmıştır. Vakaların  %66.9’u, kontrollerin ise %37.0’ı mahalle çeşme suyu (TRRMH=4.3 ,%95 GA:2.2-8.6), vakaların %18’i kontrollerin %27’si şişelenmiş su içmiştir (TRRMH=0.5, %95 GA:0.3-0,9). Beş adet dışkı örneğinde Norovirüs GI ve GII saptanmıştır. Çevresel araştırmalarda ilçede 35 kontrolsüz mahalle çeşmesi olduğu, bunları besleyen su deposunun sel suyu ile tamamen dolmuş olduğu öğrenilmiştir. Su örneklerinde norovirüs tespit edilememiştir. Sonuç: Vakalarda norovirüs saptanması, korunaksız mahalle çeşmesi deposunun taşkın yaşanan kanalın hemen yanında bulunması ve çalışmada elde edilen bulgular, salgının olası nedeninin şiddetli yağış sonrasında kontamine olan mahalle çeşmelerinden su tüketilmesi olduğunu düşündürmektedir. Purpose: Public health officials of the Rabbit District on 18.06.2014 that 28 of them were hospitalized and treated with 684 gastroenteritis patients. The examination was carried out in order to determine the cause of the epidemic, to determine the path of infection and to implement control measures. Method: In this case-control study, between June 07-30, R11, K52, A09 ICD-10 diagnostic code, vomiting or swelling (≥3/day) with a high attack rate in 5 neighborhoods, 154 possible facts and controls selected by a simple random method were studied. The controls, 1:1, were selected from the neighbors by matching the age group. Clinical samples and net water samples have been studied. Results: The outbreak rate is 3.3%, and the cases are spread throughout the district. The main complaints are nausea (%88.9), diarrhea (%88.2), stomach pain (%82. 8) and it is vomiting (%71.8). Cases began to increase 6 days after the flood and a wide pic was detected in the wing of the epidemic. In 66.9 percent of cases, and in 37.0 percent of controls, the neighboring source water (TRRMH=4.3 , 95 percent GA:2.2-8.6), in 18 percent of cases, 27 percent of controls were drunk bottled water (TRRMH=0.5, 95 percent GA:0.3-0,9). In five examples of ejaculation, Norovirus GI and GII were detected. Environmental research has found that there are 35 uncontrolled neighborhood flows in the district, the water warehouse that feeds them is completely filled with flood water. Norovirus was not detected in water samples. The result: the detection of the norovirus in cases, the unprotected neighboring fountain deposits are located right next to the channels that are affected and the findings obtained in the study suggest that the possible cause of the epidemic is the consumption of water from neighboring fountains that are contaminated after severe rainfall.
