Giriş: Status epileptikus, çocuk yoğun bakım yatışlarının önemli bir kısmını oluşturmaktadır. Bu çalışmada ünitemize konvülzif status epileptikus nedeniyle yatırılan olguların genel özelliklerinin incelenmesi ve klinik ve demografik özelliklerinin paylaşılması amaçlanmıştır. Yöntemler: Bu çalışmaya 131 olgu dahil edildi. Olguların kimlik bilgileri, akut ve süreğen hastalık varlığı, yoğun bakım ve hastane yatış gün sayıları, ilk nöbet şekli, nereden kabul edildiği, yoğun bakım takibi sırasında geçirilen nöbet sayısı, tedavide kullanılan antiepileptikler ve uygulanan diğer tedaviler ve taburculuk-mortalite durumları kaydedildi. Bulgular: Olguların %62,6’sı erkekti ve tüm olguların yaş ortalaması 53,69±47,14 ay’dı. En sık görülen süreğen hastalık epilepsi, en sık görülen akut hastalık alt solunum yolu enfeksiyonu olarak bulundu. Olguların %34,4’ünün ilk başvuru şekli status epileptikus şeklinde olup, başka hastanelerden kabul edilen olgu oranı %60,3 idi. Yoğun bakım takibi sırasında olguların %77,1’inde konvülziyon görülmedi. İlk tedavide %77,1 oranında benzodiyazepin infüzyonu kullanıldı. Benzodiyazepin infüzyonu kullanılan grupta yoğun bakım yatış süresi medyan 4 (3-8) gün, kullanılmayan grupta 3 (2-3) gün’dü. İkinci sırada tercih edilen antiepileptikler %62,6 oranında levatirasetam ve %39,7 oranında fenitoin oldu. Mortalite oranımız %2,3 olarak bulundu. Sonuç: Çalışmamızda, beş yıllık süre zarfında çocuk yoğun bakım ünitemize konvülzif status epileptikus nedeniyle yatırılan olguların büyük çoğunluğunun erkek cinsiyet olduğu; süreğen nörolojik hastalığı olan çocuklarda daha sık görüldüğü, benzodiyazepin infüzyonunun ilk tercih edilen tedavi şekli olduğu ve ikinci sırada en çok kullanılan antiepileptiğin Levatirasetam olduğu görülmüştür.
Introduction: Status epileptic constitutes a significant part of the child’s intensive care stages. This study aims to study the general characteristics of the incidents deposited due to the convulsive status epileptic and to share the clinical and demographic characteristics of our unit. Methods: 131 facts were included in this study. The identity of the incidents, the presence of acute and persistent disease, the number of days of intensive care and hospitalization, the first form of the attack, where it was accepted, the number of attacks passed during intensive care tracking, the antiepileptic drugs used in treatment and other therapies applied and the cases of departure-mortality were recorded. Results: 62.6% of cases were male and the average age of all cases was 53.69±47.14 months. The most common disease epilepsy, the most common acute disease, was found as sub- respiratory infection. In 34.4 percent of cases, the first form of application was in the status epileptic form, and the rate of cases accepted from other hospitals was 60.3 percent. During intensive care tracking, there was no convulsion in 77.1% of cases. In the first treatment, 77.1% of benzodiazepine infusion was used. In the group used for the infusion of benzodiazepine, the intensive maintenance period was 4 (3-8) days, and in the group unused 3 (2-3) days. In the second place, the preferred antiepileptics were 62.6 percent levatirasetam and 39.7 percent phenitoin. Our mortality rate was 2.3%. Result: In our study, the majority of the incidents placed in our child intensive care unit during the period of five years due to the convulsive status epileptic are male; the most common in children with constant neurological disease, the infusion of benzodiazepine is the first preferred form of treatment and the second most used antiepileptic is Levatirasetam.
