Amaç: Meme radyoterapisinden (RT) sonra tiroid bezinde radyasyona bağlı ikincil malignite gelişme riski artmaktadır. Bu çalışmada yoğunluk ayarlı radyoterapi (Intensity modulated radiotherapy-IMRT) yapılan meme kanseri hastalarında tiroid bezi malignitesi gelişme riskinin değerlendirilmesi amaçlanmıştır. Yöntem: Çalışmada total mastektomi ve meme koruyucu cerrahi sonrası IMRT uygulanan 70 meme hastası değerlendirilmiştir. Hastalarda tiroid bezinde ikincil malignite gelişme riski, ERR (Estimating relative risk-tahmini rölatif risk) modeli kullanılarak hesaplanmıştır. Bulgular: Supra klaviküler bölgenin, RT alanına dahil edildiği hastalarda (Grup-1), ERR değeri supraklaviküler bölgenin RT alanına dahil edilmediği hastalardan (Grup-2) daha yüksek olduğu görülmüştür. ERR değerleri, yaş arttıkça eksponansiyel olarak azalmıştır. Gruplar arasındaki tiroid bezi dozları farklı olmasına rağmen, yaş arttıkça iki gruptaki ERR değerleri birbirine yaklaşmıştır. Sonuç: Meme IMRT’si sonrası, tiroid bezinde ikincil malignite gelişme riski, genç hastalarda daha yüksektir. Bu nedenle genç hastalardaki RT uygulamalarında sağlam ve geçerli risk değerlendirmesi tavsiye edilmektedir.
Purpose: After breast radiation therapy (RT), the risk of radiation-related secondary malignity development in the thyroid gland increases. In this study, intensity modulated radiotherapy (IMRT) is aimed at assessing the risk of developing thyroid malignity in breast cancer patients. Method: In the study total mastectomy and breast protection surgery after IMRT applied 70 breast patients were evaluated. The risk of developing secondary malignity in the thyroid gland is calculated using the ERR (Estimating relative risk-relevant risk) model. Results: In patients in which the supra clavicular area is included in the RT area (Group-1), the ERR value has been shown to be higher than in patients in which the supra clavicular area is not included in the RT area (Group-2). The ERR values have decreased exponentially with ageing. Though the thyroid gland doses between the groups were different, the ERR values in the two groups approached each other with ageing. The result: after breast IMRT, the risk of secondary malignity in the thyroid gland is higher in young patients. Therefore, a solid and valid risk assessment is recommended in RT applications in young patients.
