Amaç: Yenidoğan bebeklerde doğum sonrası yaşamın ilk 48 saatinde kan şekeri bakılma nedenleri, hipoglisemi gelişiminde risk faktörlerinin ve izlem sonuçlarının değerlendirilmesi amaçlandı. Metod: Hastanemizde 2015-2017 (üç yıl) yılları arasında doğan, yaşamın ilk 24 saati içerisinde kan şekeri düzeyi bakılan ve 48 saatlik izlem sonuçları olan tüm bebekler çalışmaya alındı. Bebeklerin dosyalarından; gebelik süresi, doğum ağırlığı, cinsiyet, antenatal, natal ve postnatal özellikler, kan şekerinin bakılma nedeni ile yöntemi, kan şekerinin bakılma zamanı, kan şekeri sonucu ve sonucuna göre yapılan izlem değerleri, bebeklerin beslenme durumları, kan şekeri düşüklüğü nedeniyle yatış gereksinimi ve kan şekerleri düşük saptanan bebeklerin 48 saat boyunca izlem bulguları kaydedildi. Çalışmaya dahil edilen bebekler grup olarak diyabetik anne bebeği olma, SGA (small for gestational age), LGA (large for gestational age), geç preterm olma (34-36+6/7 GH), fetal distres ve beslenme intoleransı durumuna göre ayrıldı. Gruplar içerisinde kan şekeri ölçüm değerleri ve nedenleri karşılaştırıldı. Bulgular: Çalışma toplam 9480 bebek ile tamamlandı. Bebeklerin %28,7’sine (n: 2720) kan şekeri değeri bakıldığı belirlendi. Bebeklerin ortalama doğum ağırlığı ve gebelik süresi; 3143±804 gram ve 37,7±2,5 hafta, cinsiyet dağılımı %54,7 erkek, doğum şekli %57,5 sezaryan şeklindeydi. Kan şekeri ölçümü için en sık ön görülen risk faktörlerinin LGA (%25,9), prematüre (%18), geçici taşipne (%17,3) ve SGA (%11,6) özelliklerinin olduğu görüldü. İzlemde 2009 bebeğin (%73,9) kan şekeri değerlerinin normal sınırlarda olduğu için anne yanında tekrar kan şekeri bakmadığı saptandı. Bebeklerin 711’inde (%26,1) kan şekeri düşüklüğü saptandığı için kan şekeri takibine alındığı belirlendi. Hipoglisemi ön tanısıyla yatış gereksinimi sıklığı %2.5 (n: 67) saptandı. Grup analizlerinde birden fazla nedeni olanların 1.saat kan şekeri değeri ortalaması, geçici taşipne ve fetal distres olanlara göre (p<0,001), Prematürite ve LGA olan bebeklerin 6.saat kan şekeri ortalamaları, diyabetik anne bebekleri kan şekeri ortalamalarına göre istatistiksel olarak anlamlı düşüktü (p<0,001). Sonuç: Doğum sonrası dönemde yenidoğan bebeklerde kan şekeri bakılma oranının %28,7 oranında olduğu ve en sık ön görülen risk faktörünün LGA bebekler olduğu saptandı. Hipoglisemi nedeniyle doğum sonrası yatış sıklığının %2,5 olduğu, birden fazla nedenin birlikte olduğu gruplarda birinci saat kan şekerinin daha düşük olduğu görüldü.
Objective: The first 48 hours of postpartum life in newborn babies were aimed at assessing the causes of blood sugar care, risk factors in the development of hypoglycemia and the monitoring results. Method: All babies born between the years 2015-2017 (three years) in our hospital were taken to study with blood sugar levels in the first 24 hours of life and 48 hours of monitoring results. From the records of babies; pregnancy time, birth weight, gender, antenatal, natal and postnatal characteristics, the treatment cause and method of blood sugar, the time of treatment of blood sugar, blood sugar outcome and outcome follow-up values, the nutritional conditions of babies, the need for sleep due to low blood sugar and the monitoring findings of babies with low blood sugar detected for 48 hours. The babies included in the study were grouped by being a diabetic mother baby, SGA (small for gestational age), LGA (large for gestational age), late preeterm (34-36+6/7 GH), fetal distraction and nutritional intolerance. The blood sugar measurements and causes were compared among groups. The study was completed with a total of 9480 babies. It was estimated that 28.7 percent of babies (n: 2720) had a blood sugar level. The average birth weight and pregnancy period of babies was 3143 ± 804 grams and 37.7 ± 2.5 weeks, the sex distribution was 54.7 percent male, and the birth form was 57.5 percent sezaryan. The most common risk factors for blood sugar measurements were LGA (25.9 percent), premature (18 percent), temporary tachipne (17.3 percent) and SGA (11.6 percent). The 2009 survey found that the baby’s blood sugar levels (73.9) were at normal limits, and that the mother did not look at the blood sugar again. In 711 babies (26.1%), blood sugar was found to be low. With the pre-diagnosis of hypoglycemia, the frequency of the deposit requirement was established by 2.5 % (n: 67) . The average blood sugar value of those with more than one cause in the group analysis was 1 hour, compared to those with temporary tachypne and fetal distres (p<0,001), the average blood sugar rate of babies with prematurity and LGA was 6 hour, and the average blood sugar rate of babies with diabetes was statistically significantly lower (p<0,001). The result was that the blood sugar treatment rate in newborn babies in postpartum period was 28.7%, and the most common risk factor was LGA babies. The hypoglycemic rate of postpartum sleep was 2.5% due to hypoglycemia, and the blood sugar was lower in the first hour in groups with more than one cause.
