Amaç: Bu çalışmada femur kırığı nedeniyle total kalça protezi planlanan ve Amerikan Anesteziyoloji Derneği (ASA) Sınıf ≥III olan 65 yaş üzeri hastalarda iki santral blok yöntemi olan sürekli spinal anestezi ile kombine spinal-epidural anestezi kıyaslandı. Hastalar ve yöntemler: Çalışmaya toplam 40 ardışık hasta dahil edildi. Hastalar sürekli spinal anestezi grubu (Grup 1; n=20) ve kombine spinal-epidural anestezi grubu (Grup 2; n=20) olarak iki gruba ayrıldı. Santral blok girişimleri L3-4 aralığından yapıldı. Grup 1’e spinal kateterden 2.5 µg %0.5 izobarik bupivakain verildi. Grup 2’ye 7.5 µg %0.5 izobarik bupivakain ile spinal anestezisi yapılarak epidural kateter takıldı. Blok etkinliği, Bromage ve pinprick testleri ile değerlendirildi ve pik süreleri kaydedildi. Ameliyat sırasında ve sonrasında vital bulgular ve motor ve duyusal blok seviyeleri ölçüldü ve Görsel Analog Ölçeği (GAÖ) ve Ramsey Sedasyon Ölçeği (RSS) skorları kaydedildi. Bulgular: Çalışma grupları arasında demografik özellikler açısından istatistiksel olarak anlamlı bir fark yoktu. Grup 2’nin ameliyat öncesi ve sonrası kalp atım hızında istatistiksel olarak anlamlı bir fark saptandı. Gruplar arasında motor ve duyusal blok pik süreleri, duyusal blok seviyesi ve motor blok dereceleri arasında istatistiksel olarak anlamlı bir fark izlendi. Grupların ameliyat sırası ve sonrası GAÖ ve RSS skorları arasında istatistiksel olarak anlamlı bir fark bulundu. Sonuç: Sürekli spinal anestezi, geriatrik hastalarda, yüksek sempatik blok ile gelişen hemodinamik bozuklukların azaltılmasında güvenli bir anestezi tekniği olup, derlenme dönemi daha kısa ve komplikasyonsuz olmaktadır.
Purpose: This study compared total spinal-epidural anesthesia with continuous spinal anesthesia, which is two central block methods in patients over the age of 65 and the American Anesthesiology Association (ASA) class ≥III, which was planned for total spinal protection due to femur break. Patients and methods: A total of 40 consecutive patients were included in the study. Patients were divided into two groups: the continuous spinal anesthesia group (Group 1; n=20) and the combined spinal-epidural anesthesia group (Group 2; n=20). The central block initiatives were made from the L3-4 range. Group 1 was given 2.5 μg of 0.5 % isobary bupivakain from the spinal catheter. The group 2 was infected with 7.5 μg of 0.5 % isobary bupivakain with spinal anesthesia and epidural catheter. Block effectiveness was assessed by Bromage and pinprick tests and pic times were recorded. During and after the operation, vital findings and motor and sensual block levels were measured and the scores of the Visual Analog Scale (GAÖ) and Ramsey Sedation Scale (RSS) were recorded. Results: There was no statistically significant difference in demographic characteristics between the working groups. A statistically significant difference in the pre- and post-operative heart rate was found in Group 2. A statistically significant difference was observed between the group’s engine and sensory block pic times, sensory block level and motor block degrees. A statistically significant difference was found between the group’s operational order and post-GAÖ and RSS scores. The result: Continuous spinal anesthesia is a safe anesthesia technique in the reduction of hemodynamic disorders developing with high sympathic blocks in geriatric patients, and the period of compilation is shorter and complicated.
