Giriş ve Amaç: Perkütan endoskopik gastrostomi uzun süreli enteral nütrisyona gereksinim duyan hastalarda beslenme için tercih edilen yöntemdir. Bu çalışmada perkütan endoskopik gastrostomi uygulanan hastalarımızdaki kısa ve uzun dönem sonuçlarımızı araştırdık. Gereç ve Yöntem: Bu çalışma Haziran 2017-Mayıs 2019 tarihleri arasında hastanemiz endoskopi ünitesinde perkütan endoskopik gastrostomi uygulanan hastalar ile yapıldı. Hastaların perkütan endoskopik gastrostomi endikasyonları, işlem detayları, takip süreleri ve gelişen komplikasyonlar retrospektif olarak hasta dosyalarından kaydedildi. Bulgular: Çalışmaya dahil edilen 58 hastanın 34’ü (%58.6) erkek, 24’ü ise (%41.4) kadındı. Hastaların ortalama yaşları 68.4 yıl idi (18-94). Üç hasta dışındaki 55 hastaya perkütan endoskopik gastrostomi başarıyla uygulandı (%94.8). En sık perkütan endoskopik gastrostomi yerleştirme endikasyonu nörolojik hastalıklardı (%84.4). Diğer endikasyonlar ise multitravma, malignite ve septisemiydi. Larenks kanseri nedeniyle total larenjektomili bir hastada özofagus darlığı nedeniyle balon dilatasyonu yapıldıktan sonra perkütan endoskopik gastrostomi yerleştirildi. Perkütan endoskopik gastrostomi prosedürüne bağlı olarak 4 hastada minör komplikasyonlar gözlendi (%7.3). Mortalite dahil hiçbir hastada majör komplikasyon görülmedi. Perkütan endoskopik gastrostomi prosedürü sonrası bir aylık dönem içinde 5 hastada (%8.6), bir aydan uzun dönemde ise 27 hastada diğer sebeplere bağlı mortalite gelişti (%46.6). Ölen 32 hastanın ortalama takip süresi 82 gün iken (4-355) kalan 23 hastanın ortalama takip süresi ise 279 gün idi (18-657). Sonuç: Perkütan endoskopik gastrostomi beslenme desteğinin enteral olarak sağlanmasında güvenli, gereğinde yatak başında kolayca uygulanabilen, minimal invaziv, iyi tolere edilen ve düşük komplikasyon oranına sahip bir uygulamadır.
Introduction and Purpose: Percutaneous endoscopic gastrostomy is the preferred method for nutrition in patients who require long-term enteral nitritisone. In this study, we looked at our short and long-term results in our patients with percutaneous endoscopic gastrostomy. This study was conducted between June 2017 and May 2019 with patients with percutaneous endoscopic gastrostomy in our hospital’s endoscopic unit. Patients' percutaneous endoscopic gastrostomy indications, process details, tracking times and developing complications were recorded retrospectively from the patient's files. Results: 34 (58.6%) of the 58 patients involved in the study were male and 24 (41.4%) female. The average age of patients was 68.4 years (18-94). Percutaneous endoscopic gastrostomy was successfully applied to 55 patients except for three patients (94.8%). The most common indication for the placement of percutaneous endoscopic gastrostomy was neurological diseases (84.4%). Other indications were multitravma, malignity and septism. A patient with total larenectomy due to larynx cancer was placed in percutaneous endoscopic gastrostomy after balloon dilatation due to ozofagus narrowness. Depending on the procedure for percutaneous endoscopic gastrostomy, minor complications were observed in 4 patients (7.3%). No major complications were observed in any patient, including mortality. During a monthly period after percutaneous endoscopic gastrostomy, 5 patients (8.6%) developed mortality due to other causes (46.6%) and over a month 27 patients developed mortality due to other causes (46.6). The average tracking time of the 32 dead patients was 82 days (4-355) while the average tracking time of the remaining 23 patients was 279 days (18-657). Result: Percutaneous endoscopic gastrostomy is an application that is safe in the provision of nutritional support in the enteral, which can be easily applied at bed, minimal invasive, well tolerated and with a low rate of complications.
