Amaç: Bu çalışmada, kliniğimizde sekonder hiperparatiroidizm (HPT) olgularına uygulanan subtotal paratiroidektomi ve kriyoprezervasyonsuz total paratiroidektomi ve ototransplantasyon cerrahi yöntemlerinin erken ve geç dönem sonuçları ve etkinliklerinin karşılaştırılması amaçlanmıştır. Gereç ve Yöntem: Çalışmaya paratiroidektomi yapılan 68 sekonder HPT hastası dahil edildi ve hastalar retrospektif olarak değerlendirildi. Subtotal paratiroidektomi yapılan hastalar grup 1 (n=47) olarak, total paratiroidektomi ve ototransplantasyon yapılan hastalar grup 2 (n=21) olarak sınıflandırıldı. Hastaların demografik ve klinik özellikleri değerlendirildi. Bulgular: Ameliyatta, grup 1’de 8 hastaya (%14.9), grup 2’de 17 hastaya (%76.2) eş zamanlı timektomi yapıldı. Grup 1’de görülen persistan hiperparatiroidi (HPT) oranı %14.9 (n=7) iken, grup 2’de %4.8 (n=1) idi. Rekürren HPT grup 2’de görülmezken, grup 1’de %10.6 oranında (n=5) görüldü. Tüm hastalar değerlendirildiğinde, persistan veya rekürren HPT gelişen hastaların %69.2’sinde (n=9) neden, ektopik veya süpernumara bez varlığıydı. Sonuç: Son dönem böbrek yetmezlikli sekonder HPT tanılı hastalara, bilateral servikal timektominin eklendiği, kriyoprezervasyonsuz total paratiroidektomi ve ototransplantasyon tekniği, güvenli ve etkili bir cerrahi seçenek olarak uygulanabilir.
Purpose: This study aims to compare the early and late period results and effectiveness of subtotal parathyroidectomy and cryopreservation-free total parathyroidectomy and autotransplantation surgical methods applied to secondary hyperparathyroidism (HPT) in our clinic. The study included 68 secondary HPT patients with parathyroidectomy and the patients were evaluated as retrospective. Patients with subtotal parathyroidectomy were classified as group 1 (n=47) and patients with total parathyroidectomy and autotransplantation were classified as group 2 (n=21) The demographic and clinical characteristics of the patients were assessed. Results: During the operation, 8 patients in group 1 (14.9 percent), 17 patients in group 2 (76.2 percent) were simultaneously timectomy. The rate of persistent hyperparathyroid (HPT) observed in Group 1 was 14.9 % (n=7) while 4.8 % (n=1) in Group 2. Recurren HPT was not observed in group 2, while in group 1 it was observed at 10.6% (n=5). When all patients were evaluated, 69.2 percent of persistent or recurrent developing HPT patients (n=9) were caused by ectopic or supernumber gland presence. Result: In patients with secondary HPT diagnosed with renal failure in the last period, a cryopreservation-free total parathyroidectomy and autotransplantation technique, with bilateral cervical timectomy added, can be applied as a safe and effective surgical option.
