Üriner yolda en sık görülen mesane kanseri nedeniyle radikal sistektomi uygulanan hastalarda idrarın vücuttan atılmasını sağlayan geçici veya kalıcı olabilen yeni bir yolun oluşturulması (ürostomi) gerekmektedir. Bu çalışmada, radikal sistektomi sonrası stomayı kabullenemeyen bir olgunun Roy Uyum Modeline temellendirilmiş hemşirelik bakımı tartışılmıştır. Mesane kanseri tanısı konulan hastaya üriner diversiyon uygulanarak kalıcı ürostomi açılmıştır. Hastanın ameliyat sonrası tedavi sürecine uyumda zorlandığı, ürostomi bakımını sadece eşinin yaptığı, hastanın o bölgeye hiç bakmadığı farkedilmiştir. Olgunun bakımı, Roy Uyum Modeline temellendirilerek planlanmıştır. Hastanın ürostomi gibi yaşam sürecini doğrudan etkileyen bir durumla karşı karşıya kalması, tedavi sürecine adaptasyonunu zorlaştırmaktadır. Bu bağlamda, hemşirelik bakımının bireysel özelliklere ve ihtiyaçlara göre uygun bir model ışığında planlanması önem kazanmaktadır.
After radical cystectomy performed in the advanced stage of bladder cancer, which is the most common urinary tract cancer, a new pathway, which can be temporary or permanent, is created by performing urinary diversion. In this study, nursing care based on the Roy Adaptation Model of a patient who could not accept a stoma after radical cystectomy was discussed. Permanent urostomy was opened to the patient diagnosed with bladder cancer.It was noticed that the patient had difficulty in adapting to the postoperative treatment process.The care of the case was planned based on the Roy Adaptation Model. Urinary diversion makes it difficult for the patient to adapt to the treatment process. In this context, it is important to plan nursing care in the light of an appropriate model according to individual characteristics and needs.
