Diyarbakır’da Bir İlçe Devlet Hastanesi Engelli Sağlık Kuruluna Başvuran Hastalarda Zihinsel, Ruhsal, Davranışsal Bozuklukların Değerlendirilmesi

Abstract:

Amaç: Engellilik ülkemizde önemli bir sorun olmasına rağmen bu konuda yapılmış az sayıda çalışma bulunmaktadır. Bu çalışmanın amacı ülkenin kırsal kesimine hizmet veren bir ilçe devlet hastanesinden engelli sağlık kurulu raporu alan kişilerdeki psikiyatrik tanıların sıklığının ve psikiyatrik tanı almış olan kişilerin demografik özelliklerinin ortaya konulmasıdır. Yöntemler: Eylül 2016-Şubat 2019 tarihleri arasında bir ilçe devlet hastanesi engelli sağlık kuruluna alınmış olan 3842 kişinin dosyaları geriye dönük olarak araştırıldı. Psikiyatrik bir tanı ile engel oranı almış olan 576 kişinin demografik bilgileri, engel oranları ve tanı sıklıkları incelendi. Bulgular: Sağlık kurulunda rapor düzenlenen kişilerden psikiyatrik bir tanı ile engel oranı almış olan 576 kişinin 312’sinin erkek (%54,2), 264’ünün kadın (%45,8) olduğu ve yaş ortalamalarının 42,6±17,4 olduğu saptandı. Çalışmaya alınan kişilerin toplam engel oranı ortalaması %62,7±20,9 iken, psikiyatri ile ilişkili aldıkları engel oranı ortalaması %42,5±22,5 olarak bulundu. Engelli sağlık kurulu tarafından günlük yaşam aktivitelerini başkalarının yardımı olmaksızın yerine getiremeyeceği için ‘ağır engelli’ olduklarına karar verilen kişi sayısı 96 (%0,2) idi. En sık engel oranı alınan ilk beş psikiyatrik tanı şizofreni-tedaviye rağmen çalışma olanağı olmayan (%17,4), yaygın anksiyete bozukluğu-tedavi ile işlevselliği kısmen düzelen (%17,4), hafif zeka işlev bozukluğu (IQ: 50-69) (%12,5), sınırda mental kapasite (IQ: 70-80) (%11,7) ve distimi-tedavi ile işlevselliği kısmen düzelen (%8,7) idi. Sonuç: Çalışmamıza göre sağlık kurulu başvuruları arasında psikiyatrik engelliliğe en sık yol açan hastalıklar şizofreni ve yaygın anksiyete bozukluğudur. Bu çalışma ülkemizdeki zihinsel, ruhsal ve davranışsal açıdan engelli olan kişilerin durumunu ortaya koyması açısından faydalı ve engelliler ilgili yapılacak düzenlemeler için yol gösterici olabilir.

Objective: Despite the fact that disability is a major problem in our country, there is a small number of work done on this. The aim of this study is to reveal the frequency of psychiatric diagnoses in people receiving a disabled health council report from a district state hospital that serves the rural part of the country and the demographic characteristics of people who have received psychiatric diagnoses. Methods: The files of 3842 people who were taken to the disabled health council of a district state hospital between September 2016-February 2019 were recurrently investigated. A psychiatric diagnosis examined the demographic information, the obstacle rates and the diagnosis frequencies of 576 people who received the obstacle rate. The findings: 576 of those who received a psychiatric diagnosis with a disorder rate from those who were in the Health Council found that 312 were male (54.2 percent), 264 were female (4.5 percent) and the average age was 42.6±17.4. The total obstacle rate of those who were studied was 62.7 ± 20.9%, while the average obstacle rate of those who were studied was 42.5 ± 22.5%. The number of people who were determined to be 'heavy disabled' because they could not carry out their daily life activities without the help of others was 96 (0.2 percent). The first five psychiatric diagnosis received the most frequent obstacle ratio was unable to work despite schizophrenia-treatment (17.4%), common anxiety disorder-treatment with functionality partially corrected (17.4%), light intelligence functional disorder (IQ: 50-69) (12.5%), limited mental capacity (IQ: 70-80) (11.7%), and functionality partially corrected with dystimi-treatment (8.7%). Result: According to our study, among the medical council applications, the diseases that lead to psychiatric disability are schizophrenia and common anxiety disorder. This study can be useful in the point of exposing the state of persons with mental, mental and behavioral disabilities in our country and can be a guide to the arrangements to be made in relation to persons with disabilities.

Method: The files of 3842 people who applied to a district state hospital’s disability board between September 2016 and February 2019 were retrospectively investigated. Demographic information, diagnoses and disability rates of 576 individuals who were given a psychiatric disability rate were examined. Results: Of the individuals who had been given a psychiatric diagnosis and disability rate, 312 were male (54.2%), 264 of them were female (45.8%), and the mean age was 42.6±17.4. The mean total disability rate was 62.7±20.9% and the rate of disability related to psychiatry was 42.5±22.5%. The number of people who were decided by the disability board to be severely disabled since they could not perform their daily activities without the help of others was 96 (0.2%). The first five psychiatric diagnoses with the highest rate of disability were schizophrenia- treatment resistant (17.4%), generalized anxiety disorder- partial response (17.4%), mild mental retardation (IQ:50-69) (12.5%), borderline mental capacity (IQ: 70-80) (11.7%) and dysthymia- partial response (8.7%). Conclusions: Our study revealed that the most common causes of psychiatric disability are schizophrenia and generalized anxiety disorder. This study may be useful in revealing the situation of mentally and behaviorally disabled people in our country and may also serve as a guide in making arrangements regarding these people.
