Amaç: Septorinoplasti ameliyatı hem estetik hem de fonksiyonel problemleri çözmeyi amaçlayan en sık yapılan yüz estetik girişimidir. Septorinoplastide özellikle septal eğriliklerin tedavisi fonksiyonel başarıyı etkileyen en önemli basamaktır. Septal eğriliklerin tedavisinde literatürde skorlama, kıkırdak ve kemik greft ile splintleme gibi çeşitli cerrahi teknikler önerilmiştir. Bu çalışmada kaudal septal eğriliği olan hastalarda septorinoplasti sırasında bu eğriliğin tedavisinde etmoid kemik perpendiküler lamina splinti kullanılan hastaların postoperatif fonksiyonel sonuçlarını NOSE anketi ile ortaya koymayı amaçladık. Materyal - Metod: Retrospektif bu çalışmaya kaudal septal deviasyonu ve kırığı olan 25 hasta dahil edildi. Olguların operasyondan bir hafta önce ve operasyondan bir yıl sonra uygulanan NOSE anketinin sonuçları istatistiki olarak değerlendirildi. Bulgular: NOSE ölçeğinde yer alan tüm skorlarda ameliyat sonrası istatistiksel olarak önemli derecede düşüş izlendi. Tüm hastalarda ameliyat öncesi toplam NOSE skoru ortalaması 16,84 ± 2,39 (12 - 20) olarak bulunurken ameliyat sonrası toplam NOSE skoru ortalaması 3,96 ± 1,81 (1 - 8) olarak bulundu (p<0,001). Sonuç: Etmoid lamina düz bir septumun elde edilmesi için kıkırdak greftin yetersiz miktarda olduğu veya kıkırdak greftlerin deviasyona gerekli direnci gösteremediği durumlarda hem operasyon sahasından kolaylıkla elde edilebilmesi, hem deviasyona karşı yeterli direnç sağlaması, hem de kolay işlenebilir olması ile iyi bir greft alternatifidir.
Purpose: Septorinoplasty surgery is the most common face aesthetic initiative aimed at solving both aesthetic and functional problems. In Septorinoplasty, the treatment of septal curvature is the most important step that affects functional success. A variety of surgical techniques, such as scrolling, scratching and splintering with bone greft, are recommended in the literature in the treatment of septic curvature. In this study, we aimed to reveal postoperative functional results of patients using etmoid bone perpendicular lamina splinti in the treatment of this curve during septorinoplasty in patients with caudal septal curvature with NOSE survey. Material - Method: Retrospective this study included 25 patients with caudal septal deviation and fractures. The results of the NOSE survey applied a week before the operation of the incidents and a year after the operation were statistically evaluated. Results: All scores on the NOSE scale were statistically significantly decreased after surgery. In all patients, the total pre-operative NOSE score was an average of 16.84 ± 2.39 (12 - 20), while the total postoperative NOSE score was an average of 3.96 ± 1.81 (1 - 8) (p<0,001). The result: The etmoid lamina is a good greft alternative with both easy-to-accessing from the operating field, providing sufficient resistance to deviation, and easy-to-processing, in cases where the graffiti is insufficient to obtain a flat septum, or where the graffiti are unable to show the necessary resistance to deviation.
