Amaç: Çalışmanın amacı transfemoral amputasyonu bulunan bireylerde kognitif ve motor ikili görevin yürüyüşe etkisinin incelenmesiydi. Yöntem: Çalışmaya 18 yaş ve üzerinde olan, unilateral transfemoral amputasyonu bulunan kişiler dahil edildi. Bilişsel fonksiyonların değerlendirilmesi için Montreal Bilişsel Değerlendirme Ölçeği (MOBİD) kullanıldı. Yürüyüşün zaman-mesafe karakteristiklerini belirlemek için ayak izi yöntemiyle yürüyüş analizi yapıldı. Fonksiyonel performansı değerlendirmek için Zamanlı Kalk ve Yürü Testi (ZKYT) kullanıldı. Bütün değerlendirmeler ek görev olmadan, bilişsel ikinci görevle birlikte ve motor ikinci görevle birlikte tekrarlandı. İkili görevin etkisini incelemek için Tekrarlayan ölçümler varyans analizi ve Freidman testi kullanıldı. Sonuçlar: Yaş ortalaması 51,20±15,83 yıl olan 24 birey çalışmaya dahil edildi. MOBİD skoru ortalaması 23,95±2,14 idi. Yürüyüş hızı, sağlam taraf ayak açısı ve ZKYT süreleri hem motor hem bilişsel ikili görevlerde anlamlı derecede etkilenmiş bulundu. Ampute taraf adım uzunluğu, destek yüzeyi ve tempo sadece bilişsel ikili görev ile sağlam taraf adım uzunluğu sadece motor ikili görev ile anlamlı değişiklik gösterdi (p<0,05). İkili görev performans etkileri karşılaştırıldığında hız ve temponun bilişsel ikili görev esnasında daha fazla etkilendiği görüldü (p<0,05). Tartışma: Transfemoral amputasyonu bulunan bireylerde yürüyüş parametreleri bilişsel ve motor ikili görev esnasında etkilenmektedir. Yürüyüş performansının ikili görevle birlikte azalması yürüme becerisinin yeterince otomatikleşmemiş olduğunu göstermektedir, bu sonucun fizyoterapi ve rehabilitasyon uygulamaları ile olumlu etkileneceği düşünülmektedir.
The aim of the study was to study the effects of cognitive and motor bicycle tasks on walking in individuals with transfemoral amputation. Method: Persons aged 18 and older with unilateral transfemoral amputation were included in the study. The Montreal Cognitive Assessment Scale (MOBID) was used to evaluate cognitive functions. Walking analysis was conducted by footprint method to determine the time-distance characteristics of the walk. The time-leap and walk test (ZKYT) was used to evaluate functional performance. All evaluations were repeated without additional task, with the cognitive second task and the engine with the second task. Repeated measurements were used for the analysis of variance and Freidman test to study the effect of the dual task. Results: 24 individuals with an average age of 51.20±15.83 years were included in the study. The MOBID score average was 23.95±2.14. Walk speed, solid side foot angle and ZKYT times have been significantly affected in both motor and cognitive dual tasks. The lamp side step length, the support surface and the pace showed significant changes with the solid side step length only with the cognitive binary task (p<0,05). When compared to the performance effects of binary tasks, it was found that speed and speed were more affected during the cognitive binary tasks (p<0,05). In individuals with transphemoral amputation, the walking parameters are affected during the cognitive and motor binary task. The decrease in walking performance along with the dual task indicates that the ability to walk is not sufficiently automated, this result is believed to be positively affected by physiotherapy and rehabilitation practices.
