İzole Anti-Hbc Pozitifliği Saptanan Hastaların Hepatit B Aşısına Karşı İmmün Yanıtlarının Değerlendirilmesi

Hepatit B için yüzey antijeni ve antikorunun negatif, yalnızca anti-HBc total pozitifliği olan hastalar ile zaman zaman karşılaşılmaktadır. Kliniğimizde Kasım 2000- Haziran 2004 tarihleri arasında hepatit belirteçleri araştırılan 767 hastadan 39’unda (%5,8) izole anti-HBc pozitifliği saptanmıştır. İzole anti-HBc pozitifliği saptanan hastaların 14’ü kadın, 25’i erkek olup, ortalama yaş 49.8±10.33 idi. Bu hastalara 0, 1 ve 6. aylarda rekombinant hepatit B virüs aşısı uygulandı. Her aşılamadan 1 ay sonra anti-HBs titresi kontrol edildi. Hepatit belirteçleri 3. kuşak ELİSA testi ile çalışıldı. Olguların 1’inde (% 2,6) HBeAg, 10’unda (%25,6) anti-HBe ve 3’ünde (%7,7) anti-HCV pozitifliği saptandı. Birinci aşıdan sonra 20 (% 51,2) hastada, 2. aşıdan sonra 31 hastada (%86,1) ve 3. aşıdan sonra 32 hastada (% 91,4) anti-HBs düzeyi ≥10 IU/L saptandı. 4 kişi 2. ve 3. aşılamaya katılmadı, 1 kişi 3. aşılamaya katılmadı. İki (% 5,1) olguda 3 doz aşılama sonrası yeterli bağışıklık sağlanamadı. Sonuç olarak izole anti-HBc pozitif olan kişilerde aşılama ile yüksek oranda koruyucu antikor titresi sağlanmıştır. Bu hastalarda doğrudan nükleik asid testleri yapmak yerine yalnızca aşıya yanıt oluşturmayan hastalarda bakılması ile önemli oranda tasarruf sağlanabilir

An isolated anti-Hbc positive patient’s immunity response to hepatitis B vaccine assessment

Surface antigen and antibodies for hepatitis B are occasionally met with patients with negative, only total anti-HBc positive. In our clinic between November 2000 and June 2004, 39 (5.8%) of the 767 patients studied with hepatitis indicators found isolated anti-HBc positive. 14 of the patients with isolated anti-HBc positive were female and 25 male, with an average age of 49.8±10.33. These patients were given a recombinant hepatitis B virus vaccine in the months 0, 1 and 6. One month after each vaccination, anti-HBs thyroid was checked. The symptoms of hepatitis were studied with the ELISA test of the 3rd generation. In 1 of the cases (2.6%) HBeAg, in 10 (25,6%) anti-HBe and in 3 (7.7%) anti-HCV positivity were identified. 20 (51.2%) patients after the first vaccination, 31 patients (86.1%) and 3 patients after the second vaccination. After vaccination, an anti-HBs level ≥10 IU/L was detected in 32 patients (91.4 percent). 4 people did not take part in the 2nd and 3rd vaccines, 1 person did not take part in the 3rd vaccines. Two (5.1%) cases did not provide adequate immunity after 3 doses of vaccination. As a result, high levels of protective antibodies are provided with vaccination in people with isolated anti-HBc positive. Instead of conducting direct nucleic acid tests in these patients, considerable savings can be provided by the treatment of patients who do not respond to the vaccine only.
