Amaç: Staphylococcus aureus toplum ve sağlık bakım ilişkili cilt ve yumuşak doku enfeksiyonları, üriner sistem enfeksiyonu, endokardit, pnömoni, septik artrit, osteomyelit, sepsis gibi morbidite ve mortalitesi yüksek enfeksiyonlara neden olan en önemli mikroorganizmalardan biridir. Çalışmamızda hastanemiz Tıbbi Mikrobiyoloji laboratuvarına gelen çeşitli klinik örneklerden izole ettiğimiz metisiline dirençli S. aureus (MRSA) ve metisiline duyarlı S. aureus (MSSA) izolatlarının antimikrobiyallere direnç oranlarının belirlenmesi ve hastanemize ait direnç profilinin incelenmesi amaçlandı. Gereç ve Yöntem: 2018- 2019 yılları arasında izole edilen 879 S. aureus izolatı çalışmaya dahil edildi. İzolatlar konvensiyonel yöntemlerle ve MALDI-TOF MS (Bio Merieux, Fransa) ile identifiye edildi. İzolatların antimikrobiyal duyarlılık testleri European Committee on Antimicrobial Susceptibility Testing (EUCAST) kriterlerine göre Kirby-Bauer disk düffüzyon ve Gradient düffüzyon methodu ile belirlendi. Bulgular: 2018 yılında %24.2, 2019 yılında ise %22.3 olmak üzere iki yıllık sürede MRSA oranı %23.3 olarak belirlendi. S. aureus izolatlarının dağılımına bakıldığında yara kültürlerinin en sık S. aureus izole edilen klinik örnek olduğu tespit edildi. MRSA ve MSSA izolatlarında vankomisin ve linezolide direnç belirlenmedi. En yüksek direnç oranı MRSA ve MSSA izolatlarında sırasıyla %98 ve %82.8 ile penisiline karşı tespit edildi. Sonuç: Hastanemizdeki S. aureus izolatlarının direnç profillerinin bilinmesi; ampirik tedavide yol gösterici olacaktır. Ayrıca etkili enfeksiyon kontrol önlemlerinin alınması, el hijyenine dikkat edilmesi ve kısıtlı antibiyogram uygulaması MRSA enfeksiyonlarının kontrolüne katkı sağlayacaktır.
Purpose: Staphylococcus aureus is one of the most important microorganisms that causes high infections, such as skin and soft tissue infections, urinary system infections, endocarditis, pneumonia, septic arthritis, osteomyelitis, sepsis, and morbidity. Our study aimed at determining the rates of anti-microbial resistance of methysilin-resistant S. aureus (MRSA) and methysilin-resistant S. aureus (MSSA) isolates that we isolated from various clinical samples from our hospital's Medical Microbiology Laboratory and the study of the resistance profile of our hospital. Tools and Methods: The 879 S. aureus isolates, which were isolated between 2018 and 2019, were included in the work. Isolates were identified by conventional methods and by MALDI-TOF MS (Bio Merieux, France). The anti-microbial sensitivity tests were determined by the European Committee on Antimicrobial Susceptibility Testing (EUCAST) criteria using the Kirby-Bauer disc diffusion and the Gradient diffusion method. Results: In 2018, the MRSA ratio was fixed at 23.3% in the two-year period, with 24.2% and 22.3% in 2019. The distribution of S. aureus isolates was found that the wound cultures were the most frequently isolated clinical sample of S. aureus. Resistance to vancomycin and linezolide in MRSA and MSSA isolates has not been determined. The highest resistance rate was found in MRSA and MSSA isolates with 98% and 82.8% respectively against penicillin. The knowledge of the resistance profiles of the S. aureus isolates in our hospital; will be a guide in empirical treatment. Also, taking effective infection control measures, pay attention to hand hygiene and limited antibiotic application will contribute to the control of MRSA infections.
