Amaç: Yoğun bakım ünitesine (YBÜ) yeniden başvuru yaygın ve istenmeyen bir durumdur. Mortalite oranları, YBÜ’de kalış süresi ve tedavi giderleri yeniden yatırılan hastalarda daha yüksektir. Bu çalışmada, YBÜ’den taburcu edilen hastalarda hastane/YBÜ’ye yeniden başvuru oranlarının ve buna neden olan risk faktörlerinin incelenmesi amaçlanmıştır. Gereç ve Yöntem: 1 Ocak 2012 ve 31 Ekim 2016 tarihleri arasında YBÜ’de yatmış ve taburculuk sonrası hastaneye/YBÜ’ye 30 gün içinde yeniden başvuran 18 yaşından büyük hastalar üzerinde retrospektif bir analiz yapıldı. Bulgular: Toplam 510 hasta çalışmaya dahil edildi. Bunların 91’i (%17,84) YBÜ’ye yeniden başvurdu. YBÜ’ye ilk başvuru ve taburculuk sırasında hesaplanan akut fizyoloji ve kronik sağlık değerlendirmesi-II ve sıralı organ yetmezliği değerlendirmesi skorları ile taburculuk sırasında hesaplanan stabilite ve iş yükü indeksi (SWIFT) skoru ve komorbid hastalık oranları yeniden başvuran hastalarda daha yüksekti (p<0,05). Yeniden başvuran hastaların yaş ortalaması daha yüksekti (p=0,002). Mekanik ventilasyon desteği ile taburcu edilen hastaların tekrar başvuru oranı daha yüksekti (p=0,041). Risk analizi modelimizde yaş [olasılık oranı (OR), 1,02; %95 güven aralığı (CI), 1,01-1,03] ve böbrek hastalığı varlığı (OR, 5,72; %95 CI, 2,81-11,65) yeniden yatış riskini artıran faktörler olarak belirlendi. Sonuç: Taburculuk sırasında hesaplanan yüksek akut fizyolojik skorlar ile SWIFT skoru ve komorbid hastalıkların varlığı hastaneye/YBÜ’ye yeniden yatışı öngörebilir.
Purpose: Re-application to the Intensive Care Unit (YBU) is a common and unwanted situation. The mortality rates, the duration of stay and the treatment costs are higher in patients recovered. In this study, it is intended to examine the rates of re-application to the hospital/YBU in patients who have been deported from the YBU and the risk factors that cause it. A retrospective analysis was carried out on patients over the age of 18 who have been hospitalized and re-admitted to the hospital/Hospital after deportation within 30 days between 1 January 2012 and 31 October 2016. A total of 510 patients were included in the study. 91 of them (17.84) re-applied to the YBU. The acute physiology and chronic health assessment calculated during the first application and departure-II and sequential organ insufficiency assessment scores and the stability and workload index calculated during departure (SWIFT) scores and comorbid disease rates were higher in patients re-applicating (p<0,05). The average age of patients who re-applicated was higher (p=0,002). The re-application rate of patients with mechanical ventilation support was higher (p=0,041). Our risk analysis model identified the age [probability ratio (OR), 1,02; 95% confidence range (CI), 1,01-1,03] and the presence of kidney disease (OR, 5,72; 95% CI, 2,81-11,65) as factors that increase the risk of reinsertion. The result: with high acute physiological scores calculated during departure, the SWIFT score and the presence of complicated diseases can predict re-hospitalization.
