HEMODİYALİZ TEDAVİSİ ALAN KRONİK BÖBREK YETMEZLİĞİ HASTALARINDA SEMPTOM YÖNETİMİ VE HEMŞİRELİK

Abstract:

ÖZ Kronik böbrek yetmezliği, ‘Glomerüler filtrasyon hızındaki azalma sonucunda böbreğin, sıvı­elektrolit dengesini düzenleme işlevinin ve metabolik-endokrin fonksiyonlarının irreversibl olarak bozulması’ olarak tanımlanmıştır. Glomerüler filtrasyon hızının 20-25 ml/dakika ve altında olması durumunda hastalara renal replasman tedavisi uygulanmaya başlanır. En sık uygulanan renal replasman tedavisi hemodiyalizdir. Hemodiyaliz tedavisi hastanın yaşamını devam ettirebilmesi için kullanılan ve hastaların haftanın belli zaman dilimlerinde bağlı olarak yaşaması gereken bir tedavi yöntemidir. Hastalar tedavi sırasında ya da sonrasında birçok semptomla karşı karşıya kalmaktadır. Hemodiyaliz hastalarında görülen semptomlar şu şekilde sıralanmıştır; yorgunluk (%81), nefes darlığı (%71), uykusuzluk (%67), iştahsızlık (%67), bulantı-kusma (%67), baş dönmesi (%57), kuru ve kaşıntılı cilt (%52), ishal/kabızlık (%48), ağrı (%48), baş ağrısı (%43), kas krampları (%38), uyuşukluk (%38), zayıflık (%33), göğüs ağrısı (%33), ödem (%33) olarak belirlenmiştir. Hastalığın ve tedavinin sonucunda ortaya çıkan semptomlar hastaları birçok yönden olumsuz olarak etkilemektedir. Bu nedenle semptom yönetimi önem kazanmaktadır. Amacımız doğrultusunda, kronik böbrek yetmezliği tanısı ile hemodiyaliz tedavisi alan bireylerle çalışan hemşirelerin semptomlar konusunda bilgilendirmek, semptom yönetimini uygun ve etkili bakım için yol gösterici olmak amacıyla bu derleme yapılmıştır.

Chronic renal failure is the irreversible deterioration of the function of regulating the fluid and fluid metabolic function, as well as the metabolic-endocrine function of the kidney as a result of a decrease in glomerular filtration rate. Renal replacement therapy is initiated when the glomerular filtration rate is 20-25 ml / min or less. The most common renal replacement therapy is hemodialysis. Hemodialysis treatment is a treatment method that is used for the survival of the patient and the patients should live depending on the time periods of the week. Patients experience many symptoms during or after treatment. Symptoms seen in hemodialysis patients are as follows; fatigue (81%), dyspnea (71%), insomnia (67%), loss of appetite (67%), nausea-vomiting (67%), dizziness (57%), dry and itchy skin (52%), diarrhea / constipation (48%), pain (48%), headache (43%), muscle cramps (38%), drowsiness (38%), weakness (33%), chest pain (33%), edema (33%). Symptoms resulting from disease and treatment adversely affect patients in many ways. Therefore, symptom management becomes important. The aim of this study was to inform nurses who work with hemodialysis patients with the diagnosis of chronic renal failure and to guide symptom management for appropriate and effective care.
