Existence of synchronous papillary carcinoma in thyroglossal duct cyst and thyroid gland is rare. Synchronous papillary carcinoma may be thought of as a multifocal tumor or metastasis of thyroid carcinoma to the thyroglossal duct cyst. Total thyroidectomy performed with Sistrunk's operation due to the possibility of intrathyroidal carcinoma or local invasion, is an effective treatment for thyroglossal duct cyst carcinoma. In the presence of lymph node metastasis to the neck, selective neck dissection should be added to the operation. In this article, a 38-year-old woman with synchronous papillary carcinoma in thyroglossal duct cyst and thyroid gland that metastasized to the neck was presented. Total thyroidectomy and selective neck dissection accompanied by frozen section procedure was performed as treatment, as the pathologic examination of the previous Sistrunk procedure specimen was reported as papillary carcinoma.
